初产妇剖宫产后护理干预对产妇母乳喂养率的影响

Effect of Humanistic Nursing on Mood and Breast Feeding Rate of Primiparas After Cesarean Section

目的探讨初产妇剖宫产后护理干预对产妇母乳喂养率的影响。方法选取在我院住院并符合入选标准的110例初产剖宫产产妇作为受试者,随机分为对照组和观察组各55例,术后两组分别实施常规护理与人性化护理干预。采用抑郁自评量表(SDS)、焦虑自评量表(SAS)、视觉模拟疼痛量表(VAS)观察两组采取不同护理模式后不良情绪的评分和术后48h乳房胀痛评分结果对比;同时比较两组患者母乳喂养率和48h泌乳量的情况。结果术后1个月,观察组SDS、SAS评分均显著低于对照组,差异有统计学意义(P<0.05);观察组术后3天、1周及1个月后的母乳喂养率,观察组分别为98.18%、94.55%、90.90%,均显著高于对照组的87.27%、81.82%、76.36%,差异有统计学意义(P<0.05);术后48 h观察组产妇的泌乳量量多和中量以上的比率,观察组分别为61.82%和90.91%,均显著高于对照组的38.18%和74.55%,差异有统计学意义(P<0.05);术后48 h观察组产妇乳房胀痛的情况0级和3级分别为23.64%和5.45%,明显优于对照组的9.10%和23.64%,差异有统计学意义(P<0.05)。结论人性化护理干预对初产剖宫产产妇的产后情绪、母乳喂养率、乳汁分泌情况及乳房疼痛情况均有积极作用,有利于产妇和新生儿的身体健康,值得临床推广和应用。

Objective To explore the effect of humanistic nursing on the emotion and breast feeding rate of primiparas after cesarean section.Methods 110 primiparas who were hospitalized in our hospital and met the selected criteria were randomly divided into the control group and the observation group with 55 cases in each group.SDS,SAS and vas were used to observe the scores of adverse emotions and breast distension pain 48 hours after operation.At the same time,the breast feeding rate and 48 hours lactation were compared between the two groups.Results One month after operation,the SDS and SAS scores of the observation group were significantly lower than those of the control group（P〈0.05）.The breast feeding rate of the observation group was 98.18%,94.55% and 90.90% respectively,which was significantly higher than that of the control group（87.27%,81.82%,76.36%,P〈0.05）.48 hours after operation,the rate of lactation in observation group was higher than that in control group（61.82% and 90.91%,respectively）,which was significantly higher than that in control group（38.18% and 74.55%,P〈0.05）.The level 0 and level 3 of breast distension pain in the observation group were 23.64% and 5.45% respectively 48 hours after operation,which were significantly better than those in the control group（9.10% and 23.64%,P〈0.05）.Conclusion Humanistic nursing intervention has positive effects on postpartum mood,breast feeding rate,milk secretion and breast pain of primiparas undergoing cesarean section.it is beneficial to the health of parturients and newborns,and worthy of clinical promotion and application.
