异氟烷丙泊酚和依托咪酯对老年人麻醉诱导插管的血流动力学及应激反应的研究 被引量:4

摘要 目的研究探讨异氟烷丙泊酚和依托咪酯对老年人麻醉诱导插管的血流动力学及应激反应的影响。方法方便选择2017年1月—2018年1月该院老年麻醉诱导插管患者80例进行讨论,把所有患者随机分为两组各40例,其中观察组采用异氟烷丙泊酚进行麻醉诱导,对照组采取依托咪酯进行麻醉诱导,观察两组患者的血流动力学及应激反应。结果观察组采用异氟烷丙泊酚进行麻醉诱导后,血流动力学指标比对照组的要稳定。T3、T4、T5时对照组患者的皮质醇水平分别为(271.6±75.6)、(268.4±85.9)、(265.7±95.5)g/L,明显较对应时间点的观察组要高,差异有统计学意义(t=2.092、2.077、2.394,P<0.05)。结论对于老年麻醉诱导插管的患者,采取异氟烷丙泊酚进行麻醉诱导,可以帮助患者稳定血流动力学的变化,减少应激反应的出现,值得在今后的麻醉诱导中应用推广。 Objective To investigate the effects of isoflurane propofol and etomidate on hemodynamics and stress response in anesthesia-induced intubation in the elderly. Methods A total of 80 elderly patients with anesthesia-induced intubation in the hospital from January 2017 to January 2018 were convenient selected and enrolled. All patients were randomly divided into 40 groups, 409o of which were treated with isoflurane propofol. The control group was induced by anesthesia with etomidate, and the hemodynamics and stress response of the two groups were observed. Results After the anesthesia induction of isoflurane propofol in the observation group, the hemodynamic parameters were stable compared with the control group. Cortisol levels in the control group at T3, T4, and T5 were (271.6±75.6), (268.4±85.9), (265.7±95.5)g/L, respectively, which was significantly higher than that of the observation group at the corresponding time point, the difference was statistically significant (t=2.092, 2.077, 2.394, P〈0.05). Conclusion For patients with anesthesia-induced intubation, the use of isoflurane propofol for anesthesia induction can help patients stabilize hemodynamic changes and reduce the occurrence of stress response, which is worthy of application in future anesthesia induction.
