腹腔镜下三种阑尾切除方式的临床效果观察

Effects of three appendectomy methods under laparoscope

目的比较不同腹腔镜阑尾切除方式的临床效果。方法 82例决定行腹腔镜阑尾切除术的患者随机分为超声刀组(超声刀联合结扎夹法,n=28)、结扎夹组(电钩联合结扎夹法,n=27)、缝合组(缝扎联合缝合包埋法,n=27),观察三种方式的手术时间、并发症情况、住院天数及住院费用等情况。结果三组手术时间、住院天数、并发症发生情况均无显著差异(P> 0. 05);缝合组住院费用显著低于其他两组,差异有统计学意义(P <0. 05)。结论缝合结扎方式处理阑尾系膜及根部,可在不影响手术效果的前提下,降低患者的住院费用。

Objective To evaluate the clinical effects of three different methods of laparoscopic appendectomy（ LA）.Methods 82 patients of appendicitis admitted for LA were randomly divided into three groups,the first group treated with ultrasonic knife combined with ligating clip method（ ultrasound knife group,n = 28）,the second group treated with ulectrocautery combined with the ligating clip method（ ligating clip group,n = 27）,and the third group treated with suture ligation combined with suture embedding method（ suture group,n = 27）. The operation time,complications,hospital days and costs were observed and compared. Results No statistically significant differences were found between the groups in operation time,complications,and hospital days（ P〈0. 05）. The hospital costs of the suture group was significantly lower than that of the other two groups（ P〈0. 05）. Conclusions Suture ligation combined with suture embedding as a method of LA can considerably reduce the patient＇s hospital costs without any prejudice to the effect of operation.
