PCNA和CyclinD1在腮腺恶性多形性腺瘤组织中的表达和临床意义 被引量:2

Expression and Clinical Significance of PCNA and CyclinD1 in the Malignant Pleomorphic Adenoma of the Parotid Gland

摘要 目的研究增殖细胞核抗原(Proliferating Cell Nuclear Antigen,PCNA)以及细胞周期蛋白D1(Cyclin D1)在腮腺恶性多形性腺瘤组织中的阳性表达情况以及临床意义。方法使用免疫组织化学染色方法分别观察从2016年1月—2017年12月在盐城市第一人民医院病理科37例正常腮腺组织、37例多形性腺瘤组织和59例恶性多形性腺瘤组织中的PCNA和CyclinD1阳性表达情况。结果 PCNA和Cyclin D1在正常腮腺组中的阳性表达率分别为10.81%和2.77%,在多形性腺瘤组阳性表达率为45.95%和43.24%,在恶性多形性腺瘤组的阳性表达率为83.05%和86.44%,PCNA和Cyclin D1在正常腮腺组中的阳性表达率低于多形性腺瘤组阳性和恶性多形性腺瘤组在多形性腺瘤组中的阳性表达率,差异有统计学意义(P<0.05)。PCNA与Cyclin D1在肿瘤直径>4 cm、TNM分期为III,IV期、有淋巴结转移的恶性多形性腺瘤组织中的阳性表达率高于肿瘤直径≤4 cm、TNM分期为I,II期、无淋巴结转移的恶性多形性腺瘤组织中的阳性表达率,差异有统计学意义(P<0.05)。Cyclin D1与PCNA在恶性多形性腺瘤的表达成正相关(r=0.821,P<0.05)。结论 CyclinD1、PCNA蛋白在恶性腮腺混合瘤中的阳性表达明显增加,表明其异常表达与恶性腮腺混合瘤发生可能密切相关,并可能对预测恶性腮腺混合瘤预后有重要意义。 Objective To research the positive expression and clinical significance of PCNA and CyclinD1 in the ma- lignant pleomorphic adenoma of the parotid gland. Methods The PCNA and CyclinD1 positive expression conditions of 37 cases with normal parotid tissue, 37 cases with pleomorphic adenoma and 59 cases with malignant pleomorphic ade- noma in our hospital from January 2016 to December 2017 were respectively observed by the immunohistochemical staining method. Results The positive expression rates of PCNA and CyclinD1 in the normal parotid group, pleomorphic adenoma group and malignant pleomorphic adenoma group were respectively 10.81%, 2.77%, 45.95%, 43.24%, 83.05%, 86.44%, and the positive expression rate of PCNA and CyclinD1 in the normal parotid group was lower than that in the pleomorphic adenoma group and in the malignant pleomox33hic adenoma group, and the difference was statis- tically significant(P〈0.05), and the positive expression rate of PCNA and CyclinD1 in the patients whose tumor diame- ter 〉 4 cm, whose TNM staging was stage III and stage IV, with lymph node metastasis was higher than those whose tumor diameter ≤4 cm, whose TNM stage was stage I and stage II, without lymph node metastasis, and the difference was statistically significant(P〈0.05), and the PCNA and CyclinD1 were positively correlated with the expression of ma- lignant pleomorphic adenoma of the parotid gland (r=0.821, P〈0.05). Conclusion The positive expression of PCNA and CyclinD1 protein in the malignant parotid mixed tumor obviously increases, which shows that the abnormal expression may be closely related to the occurrence of malignant parotid mixed tumor, which is of important significance to pre- dicting the prognosis of malignant parotid mixed tumor.
