左卡尼汀联合血液透析治疗尿毒症性周围神经病变的临床疗效分析

Analysis of Clinical Curative Effect of Levocarnitine Combined with Hemodialysis in Treatment of Uremic Peripheral Neuropathy

目的观察分析尿毒症性周围神经病变患者采用左卡尼汀联合血液透析治疗的临床疗效。方法选取该院2015年1月—2017年1月期间收治的50例尿毒症性周围神经病变患者,按照不同治疗方式随机分为实验组(25例,采用常规血液透析治疗)和对照组(25例,采用左卡尼汀联合血液透析治疗),对比两组患者治疗总有效率以及治疗前后腓总神经、胫神经、正中神经等指标变化。结果实验组治疗有效24例,对照组治疗有效18例,实验组患者治疗总有效率96.0%显著高于对照组72.0%,两组总有效率差异有统计学意义(χ~2=6.612,P<0.05)。两组患者治疗后胫神经、正中神经、腓总神经等感觉神经传导速度相对治疗前均有所增加,但实验组患者治疗后各感觉神经传导速度更优于对照组,差异有统计学意义(t=4.791、6.709、8.634,P<0.05)。结论尿毒症性周围神经病变患者采用左卡尼汀联合血液透析治疗的临床疗效良好,可有效改善患者临床症状,有利于恢复患者感觉神经功能,可在临床上广泛推广应用。

Objective To observe the clinical curative effect of levocarnitine combined with hemodialysis in treatment of uremic peripheral neuropathy. Methods 50 cases of patients with uremic peripheral neuropathy admitted and treated in our hospital from January 2015 to January 2017 were selected and randomly divided into two groups with 25 cases in each according to different treatment ways, the experimental group used the routine hemodialysis, while the control group used the levocarnifine combined with hemodialysis, and the total treatment effective rate, and changes of peroneal nerve, tibial nerve, median nerve before and after treatment were compared between the two groups. Results 24 cases were effective and 18 cases were effective in the experimental group and in the control group, and the total effective rate in the experimental group was obviously higher than that in the control group （96% vs 72%）, and the difference was statistically significant （X==6.612,P〈0.05） , and the tibial nerve, median nerve and peroneal nerve of the two groups after treatment were improved compared with those before treatment, and the sensorT nerve conduction velocities in the experimental group after treatment were better than those in the control group, and the differences were statistically sig- nificant （t=4.791, 6.709, 8.634, P〈0.05）. Conclusion The clinical curative effect of levoearnitine combined with hemodialysis in treatment of uremic peripheral neuropathy is good, which can effectively improve the clinical symptoms of patients, which is conductive to the recovery of sensory nerve function, and it is worth wide promotion and applica- tion in clinic.
