炎性因子和氧化应激指标在糖尿病视网膜病变患者外周血中的表达 被引量:13

Inflammatory Cytokines and Oxidative Stress Markers Expression Patterns in Peripheral Blood of Patients with Diabetic Retinopathy

摘要 目的探讨炎性因子和氧化应激指标在糖尿病视网膜病变(DR)患者外周血中的表达。方法选择2017年1月至2018年1月就诊于青岛大学附属医院的糖尿病患者为研究对象,其中糖尿病无视网膜病变、非增生型DR、增生型DR患者各30例,另外同期选取30名健康体检者为对照组,比较各组受试者血清炎性因子、氧化应激指标及血糖、血脂水平。结果增生型DR组患者的外周血白细胞介素6(IL-6)、肿瘤坏死因子α(TNF-α)和C反应蛋白(CRP)水平最高,分别为(26. 8±1. 7) ng/L、(15. 6±1. 5) ng/L、(7. 9±0. 9) mg/L,其后依次为非增生型DR组、糖尿病无视网膜病变组、健康对照组,任意两组患者IL-6、TNF-α和CRP表达水平比较差异均有统计学意义(P <0. 05)。外周血超氧化物歧化酶(SOD)在健康对照组水平最高[(104±10) k U/L],其后依次为糖尿病无视网膜病变组、非增生型DR组、增生型DR组;丙二醛(MDA)则相反,在非增生型DR组水平最高[(21. 0±1. 8) nmol/m L],其后依次为非增生型DR组、糖尿病无视网膜病变组、健康对照组,任意两组患者外周血MDA和SOD比较差异均有统计学意义(P <0. 05)。增生型DR组、非增生型DR组、糖尿病无视网膜病变组患者的血清糖化血红蛋白、空腹血糖、总胆固醇、低密度脂蛋白胆固醇水平显著高于健康对照组(P <0. 05)。结论 DR患者体内存在炎性因子的高表达以及氧化应激指标的异常,在增生型DR患者中更为明显。 Objective To explore inflammatory cytokines and oxidative stress markers expression patterns in peripheral blood of diabetic retinopathy(DR) patients. Methods According to the diagnosis, patients with diabetes admitted to the Hospital Affiliated to Qingdao University from Jan. 2017 to Jan. 2018 were divided into a non-diabetic retinopathy(NDR) group, a non-proliferative diabetic retinopathy(NPDR) group, and a proliferative diabetic retinopathy(PDR) group, each of 30 cases. And 30 healthy people undergoing physical examination during the same period were included as a control group. Serum ilfflamnlatory eytokines, oxidative stress indicators, blood glucose and blood lipids were determined of different groups were compared. Results The expression levels of interleukin-6(IL-6) ,tumor necrosis faetor-α(TNF-α) ,C-reactive protein (CRP) were highest in the PDR groupI(26.8±1.7) ng/L,(15.6±1.5) ng/L,(7.9±0.9) mg/Ll and in turn, followed by the NPDR group,NDR group and control group. The differences of IL-6,TNF-α and CRP expression between any two groups were significant ( P 〈 0.05 ). The content of superoxide disnmtase (SOD) was highest in the control group Ⅰ (104 ±10) kU/LI, and in turn, followed by the NDtl group, NPDR group and PDR group. Conversely, the level of malondialdehyde(MDA) was highest in the PDtl group I (21-0 ±1.8) nmol/mLl and in turn, followed by the NPDR group, NDR group and control group. The differences of MDA and SOD content between any two groups were significant (P 〈 0.05 ). Compared with the control group,the level of peripheral blood glyeosylated hemoglobin, fasting plasma glucose, total cholesterol and low density lipoprotein cholesterol were much higher in the three diabetic group (P 〈 0.05 ). Conclusion High expression of ilLflanmlatory eytokines and abnormal content of oxidative stress indicators are presented in DR patients, especially in the patients with PDR.
