基因突变与中国原发性骨髓纤维化临床特征及预后 被引量:1

摘要 目的研究不同基因突变类型与临床特征的相关性及其对原发性骨髓纤维化(PMF)预后的影响。方法搜集175例PMF患者的临床资料并进行随访观察记录,用巢式等位基因特异性PCR方法检测JAK2、CALR和MPL515基因突变并用Sanger方法对存在基因突变的患者CALR、JAK2和MPL515基因进行基因测序。应用Cox多因素分析的方法对175例PMF患者的生存情况进行分析。结果 175例PMF患者预计生存时间15. 2年。没有JAK2V617F/MPL/CALR突变(基因突变三阴)的PMF患者预后最差,Cox多因素分析显示,年龄>60岁(P<0. 01)、血红蛋白(Hb)<100 g/L(P=0. 042)、白细胞计数(WBC)>11×109/L(P=0. 026)、外周血幼稚细胞>1%(P=0. 048)和基因突变三阴(P=0. 001)是影响PMF生存的不良危险因素。结论基因突变、年龄、WBC、Hb、外周血幼椎细胞比例影响PMF患者预后。
