186例蒙古族成人跟骨B?hler's和Gissane's角的测量及临床意义

Measurement and clinical significance of calcaneal B?hler's and Gissane's angles in 186 Mongolian adults

[目的]测量蒙古族正常成年人跟骨X线片上B?hler's和Gissane's角,比较在年龄、性别、侧别、种族等中存在的差异,为治疗跟骨骨折提供形态学的理论依据。[方法] 2011年5月～2014年5月,从内蒙古第一附属医院、鄂尔多斯市中心医院影像学中心PACS系统中收集186例正常足部或踝关节侧位X线片,测量跟骨B?hler's角和Gissane's角。对不同年龄、性别及侧别进行统计学分析,并与以往国内外相关研究比较。[结果]共纳入186例正常成年人踝关节或足部X线片,其中男88例,女98例,年龄18～68岁,平均(39.25±13.21)岁。2名医生测量B?hler's角、 Gissane's角的相关系数分别为ICC=0.872、ICC=0.893。测得B?hler's角为(30.64±4.79)°、Gissane's角为(113.91±7.89)°。两角左、右侧差异均无统计学意义(P>0.05);男性、女性之间差异也无统计学意义(P>0.05)。B?hler's角与年龄之间存在负相关性(r=-0.179,P=0.014),Gissane's角与年龄之间存在负相关性(r=-0.187,P=0.011)。[结论]成年蒙古族正常跟骨B?hler's角和Gissane.角与汉族、维吾尔族人等存在差异,且与年龄呈负相关,但在性别、侧别间无差异性,同时观察者间一致性很好,具有一定的可重复性。

[Objective] To measure calcaneal B？hler＇s angle and Gissane＇s angle in Mongolian normal adults radiographs and compare their differences regarding to age, sex, side and race to provide a morphological basis for treatment of calcaneal fractures. [Method] From May 2011 to May 2014, a total of 186 normal foot and ankle lateral radiographs were collected for this study from PACS systems of the Inner Mongolia First Affiliated Hospital and Ordos Center Hospital. The calcaneal B？hler＇s and Gissane＇s angles were measured, and statistically analyzed among different ages, genders and sides, and compared with previous studies at home and abroad. [Results] The foot and ankle lateral radiographs were measured, which taken from normal adult Mongolian, including 88 males and 98 females aged from 18 to 68 years with an average of（39.25±13.21） years. The intraclass correlation coefficient（ICC） between two observers was 0.872 for the B？hler＇s angle, while 0.893 for the Gissane＇s angle. The B？hler＇s angle ranged from 16.8° to 42.9° with an average of（30.64±4.79）°, whereas the Gissane＇s angle ranged from86.8° to 139.4° with an average of（113.91±7.89）°. There were no statistical differences between left and right sides regarding to B？hler＇s and Gissane＇s angles（P〈0.05）. There were no statistical differences between males and females in B？hler. s and Gissane＇s angles（P〈0.05）. However, the B？hler＇s angles was negatively correlated the age（r=-0.179, P=0.014,similarly, the Gissane＇s angle also proved negatively related to the age（r=-0.187, P=0.011）. [Conclusion] The normal calcaneal B？hler＇s and Gissane＇s angles of Mongolian adults are different from Han and Uygur, and negatively correlated with age, without differences between genders and sides.
