广西孕妇碘营养状况及甲状腺功能调查 被引量:15

Investigation of iodine nutrition status and thyroid function of pregnant women in Guangxi

摘要 目的了解广西孕妇碘营养状况及其甲状腺功能水平,为防治碘缺乏病策略提供科学依据。方法在广西抽取6个市,每个市抽取1个城区和1个县,随机抽取在当地生活半年以上的孕妇共723名进行问卷调查,同时采集其家中食用盐和孕妇的尿样、血样,分别测定盐碘和尿碘、血清甲状腺激素。结果孕妇家庭中食用盐盐碘中位数为24.5 mg/kg,碘盐覆盖率为97.93%;孕妇尿碘中位数为141.4μg/L,<150μg/L(碘营养不足)的比例为55.82%。人群甲状腺功能各项检测结果显示,FT3中位数为3.80 pmol/L,异常率为11.48%;FT4中位数为12.74 pmol/l,异常率为7.33%;TSH中位数为1.560 m IU/l,异常率为8.02%;TPOAb中位数为10.75 IU/ml,阳性率为9.41%;Tg Ab中位数为3.7 IU/ml,阳性率为8.30%。常见甲状腺疾病检出情况中,低TF4血症占2.63%,亚甲减占0.41%,甲减占0.28%,亚甲亢占4.70%,甲亢占1.94%,桥本甲状腺炎占0.28%。结论当地孕妇碘营养状况欠佳,应加强重点人群碘营养水平及甲状腺功能监测,进行科学补碘持续消除碘缺乏病。 Objective To investigate the iodine nutritional status and thyroid function of pregnant women in Guangxi,and to provide scientific basis for prevention and control of iodine deficiency disorders( IDD). Methods A total of 723 pregnant women were randomly selected from 12 counties in 6 cities of Guangxi Zhuang Autonomous Region. The salt iodine,urinary iodine and blood thyroid hormone levels were assayed respectively. Results The median of salt iodine was 24.5 mg/kg,and the iodized salt coverage rate was 97.93 %.The median of urinary iodine was 141.4 μg/L,the proportion of urinary iodine level below 150 μg/L was 55.82 %.The results of thyroid function were: the median of FT3 was 3.80 pmol/L,and the abnomal rate was 11.48%; the median of FT4 was 12.74 pmol/L,and the abnormal rate was 8.02 %; the median of TSH was 1.560 m IU/L,the abrormal rate was 7.33 %; the median of TPOAb was 10.75 IU/m L,and the positive rate was 9.41 %; the median of Tg Ab was 3.7 IU/m L the abnormal and the positive rate was 8.30 %.The results of common thyroid diseases: low FT4 hyperlipidemia was 2.63 %,subclinical hypothyroidism was 0.41 %,hypothyroidism was 0.28 %,subclinical hyperthyroidism was 4.70 %,hyperthyroidism was 1.94 % and HT of local pregnant women was 0.28 %.Conclusions The iodine nutrition was in poor condition. Routine monitoring of iodine nutrition and thyroid function should be carried out,and scientific iodine supply should be sustained to reach the goal of elimination of iodine deficiency.
