分析纤维支气管镜肺泡灌洗吸痰术治疗ICU患者肺部感染的临床效果

Analysis of Clinical Effects of Fiberoptic Bronchoscopy and Alveolar Lavage in the Treatment of Pulmonary Infection in ICU Patients

目的研讨ICU肺部感染患者经纤维支气管镜(简称纤支镜)下肺泡灌洗吸痰术治疗的临床价值。方法选取100例肺部感染患者参与实验观察,并按随机抽签法将患者分组,Ⅰ组(n=50)按ICU常规标准进行治疗,Ⅱ组(n=50)在上述前提下配合纤支镜下肺泡灌洗吸痰术治疗,分析评估两组治疗对临床疗效、各肺部感染症状消失时间、抗生素使用量、机械通气时间等指标的影响。结果Ⅱ组患者的临床疗效相比Ⅰ组显著优化,总有效率较Ⅰ组提高至94.0%,比较有统计学意义(P<0.05)。与Ⅰ组比较,Ⅱ组患者在各肺部感染症状(发热、咳嗽等)消失时间、机械通气时间以及入住ICU时间上均显著缩短,在抗生素使用量方面显著减少,比较有统计学意义(P<0.05)。结论对ICU肺部感染患者使用纤支镜肺泡灌洗吸痰术治疗是一种可行治疗策略,能够提升ICU救治效果,加速缓解相关肺部感染症状、减少抗生素使用量,并促进患者的恢复,值得推荐。

Objective To evaluate the clinical value of bronchoalveolar lavage and sputum aspiration in ICU patients with pulmonary infection. Methods 100 patients with pulmonary infection were selected to participate in the experimental observation and the patients were grouped according to the random drawing method. Group Ⅰ（n=50） was treated according to the ICU standard. Group Ⅱ（n=50） was treated with bronchoalveolar lavage and sputum aspiration under fiberoptic bronchoscope. The effects of two groups on clinical efficacy, disappearance time of pulmonary infection symptoms, antibiotic use and mechanical ventilation time were analyzed and evaluated. Results The clinical efficacy of group Ⅱ was significantly better than that of group Ⅰ, and the total effective rate was 94.0% higher than that of group Ⅰ（P〈0.05）. Compared with group Ⅰ, the disappearance time of pulmonary infection symptoms（fever, cough, etc.）, mechanical ventilation time and ICU stay time in group Ⅱ were significantly shortened, and the use of antibiotics was significantly reduced（P〈0.05）. Conclusion Bronchoalveolar lavage and sputum aspiration with fiberoptic bronchoscope is a feasible treatment strategy for patients with pulmonary infection in ICU. It can improve the therapeutic effect of ICU, accelerate the remission of symptoms of pulmonary infection, reduce the use of antibiotics, and promote the recovery of patients.
