综合护理模式在子宫肌瘤切除病人围术期护理中的应用效果观察

Observation on the Effect of Comprehensive Nursing Mode in Perioperative Nursing Care of Patients Undergoing Hysteromyomectomy

目的探讨分析综合护理模式在子宫肌瘤切除病人围术期护理中的应用效果。方法选取2017年5月至2018年3月本院妇产科收治的90例子宫肌瘤患者随机分为实验组和对照组,实验组给予综合护理模式护理,对照组给予常规护理,比较两组患者手术、术后恢复及住院时间,护理前后HAMA、HAMD评分,术前、术后手术应激指标,术后并发症发生情况,对护理服务满意度。结果实验组患者手术、术后肛门排气、术后卧床、住院时间较对照组显著缩短,且术中出血量显著减少,实验组患者HAMA、HAMD评分下降更显著,实验组患者心率、平均动脉压、血糖水平升高程度较对照组低,实验组患者术后并发症发生率为4.44%较对照组显著降低,对护理服务满意率为97.78%较对照组显著提高,差异具有统计学意义(P<0.05)。结论综合护理模式应用于子宫肌瘤切除患者围手术期可减少对患者的创伤,减轻手术对患者的应激,促进术后康复,改善患者心理状态,降低术后并发症的发生率,提高患者对护理服务的满意度,建议在临床上推广。

Objective To explore and analyze the effect of comprehensive nursing mode in perioperative nursing of patients with hysteromyomectomy. Methods Ninety cases of uterine fibroids were randomly divided into experimental group and control group from May 2017 to March 2018. The experimental group was given comprehensive nursing mode and the control group was given routine nursing. The operation, postoperative recovery and hospitalization time, HAMA and HAMD scores before and after nursing, preoperative and postoperative nursing were compared between the two groups. Surgical stress indicators, postoperative complications, and satisfaction with nursing services. Results The operation, postoperative anal exhaust, postoperative bed rest and hospitalization time of the experimental group were significantly shorter than those of the control group, and the amount of intraoperative bleeding was significantly reduced. The scores of HAMA and HAMD in the experimental group were significantly lower than those in the control group. The heart rate, mean arterial pressure and blood glucose levels in the experimental group were lower than those in the control group. The incidence of complications was 4.44% lower than that of the control group, and the satisfaction rate of nursing service was 97.78% higher than that of the control group. The difference was statistically significant（P〈0.05）. Conclusion The application of comprehensive nursing model in the perioperative period of hysteromyomectomy patients can reduce the trauma of patients, relieve the stress of surgery on patients, promote postoperative rehabilitation, improve the psychological status of patients, reduce the incidence of postoperative complications, improve the satisfaction of patients with nursing services, and it is recommended to promote in clinical practice.
