无创呼吸机治疗COPD合并Ⅱ型呼衰的临床疗效

Clinical Efficacy of Non-invasive Ventilator in the Treatment of COPD Combined with Type Ⅱ Respiratory Failure

目的研讨慢阻肺(COPD)合并Ⅱ型呼衰患者使用无创呼吸机治疗的临床价值。方法选择我院收诊于2017年6月至2018年6月的50例COPD合并Ⅱ型呼衰患者参与实验观察,并按随机数表法分配患者入两组,对照组(n=25)接受一般性处理,观察组(n=25)在上述条件下接受无创呼吸机治疗,分析评估两组的治疗处理情况。结果与对照组治疗前数据比较,观察组各项血气指标(PaO_2、PaCO_2等)与心率值差异相当,无统计学意义(P> 0.05);但治疗后相比,观察组各项血气指标(PaO_2、PaCO_2等)与心率值均显著优于对照组,有统计学意义(P<0.05)。实验期间,两组均未观察到严重不良症状发生。结论对COPD合并Ⅱ型呼衰患者使用无创呼吸机治疗安全可靠,有确切的呼吸功能、心率改善效果,值得推荐。

Objective To investigate the clinical value of non-invasive ventilator in patients with chronic obstructive pulmonary disease（COPD） and type Ⅱ respiratory failure. Methods Fifty COPD patients with type Ⅱ respiratory failure admitted to our hospital from June 2017 to June 2018 were selected to participate in the experimental observation. Patients were divided into two groups according to the random number table method. The control group（n=25） received general treatment. The observation group（n=25） received non-invasive ventilator treatment under the above conditions. The treatment of the two groups was analyzed and evaluated. Results Compared with the pretreatment data of the control group, the blood gas indexes（PaO2, PaCO2, etc.） in the observation group were similar to the heart rate values, and there was no statistical significance（P〈0.05）. However, compared with the blood gas indexes of the observation group after treatment,（PaO2, PaCO2, etc.） and heart rate were significantly better than the control group, which was statistically significant（P〈0.05）. No serious adverse symptoms were observed in either group during the experiment. Conclusion The use of non-invasive ventilator in patients with COPD combined with type Ⅱ respiratory failure is safe and reliable, with accurate respiratory function and heart rate improvement. It is recommended.
