2015—2017年清华大学第一附属医院头孢菌素类抗菌药物应用分析

Analysis on Application of Cephalosporin Antibiotics in the First Hospital of Tsinghua University During 2015-2017

目的:了解清华大学第一附属医院(以下简称"我院")头孢菌素类抗菌药物临床应用情况,为临床合理用药提供依据。方法:通过医院信息系统收集2015—2017年我院头孢菌素类抗菌药物的出库情况,包括药品名称、总使用量和销售金额等,计算用药频度(defined daily dose system,DDDs)、限定日费用(defined daily cost,DDC)及药品销售金额排序(B)/DDDs排序(A)等数据,并进行统计分析。结果:2015—2016年我院头孢菌素类抗菌药物的销售金额随品种数的扩充而增加,但2017年出现负增长。3年来,除个别药品外,第4代头孢菌素的B/A基本接近1,同步性良好;第3代头孢菌素的销售金额构成比最高(>80%);头孢噻肟钠舒巴坦的销售金额排序均居首位;头孢呋辛、头孢噻肟钠舒巴坦的DDDs排序均居前2位;各药品的DDC变化基本不大。结论:我院头孢菌素类抗菌药物的使用结构比较合理,但仍需进一步加强对第3代头孢菌素使用指征的管控,延缓细菌耐药性的产生。

OBJECTIVE： To investigate the clinical application status of cephalosporin antibiotics in the First Hospital of Tsinghua University（ hereinafter referred to as ＂our hospital＂）,so as to provide basis for clinical rational drug application. METHODS： Application data of cephalosporin antibiotics in hospital information system during 2015-2017 were collected,and statistical analysis was conducted on the drug names,consumption amount,defined daily dose system（ DDDs）,defined daily cost（ DDC） and ranking of sales amount（ B）/ranking of DDDs（ A）. RESULTS：During 2015-2016,the consumption sum of cephalosporin antibiotics in our hospital increased with the expansion of the number of varieties,but there was a negative growth in 2017. The B/A of fourth-generation cephalosporin antibiotics basically close to 1 except for individual drugs,and the synchronization was good. The constituent ratio of consumption sum of the third-generation cephalosporin antibiotics was the higheat（ 〉80%）; and the consumption sum of cefotaxime sodium sulbactam took the lead. The DDDs of cefuroxime and cefotaxime sodium sulbactam dominated the top 2places; and the DDC of various drugs had no significant change. CONCLUSIONS： The application structure of cephalosporin antibiotics in our hospital is reasonable,but it is necessary to further strengthen the control of indications for the application of the third generation of cephalosporin,and delay the emergence of bacterial resistance.
