61例多西他赛致药品不良反应报告分析及处理对策

Analysis and Countermeasures of 61 Cases of Adverse Drug Reactions Induced by Docetaxel

目的:了解蚌埠医学院第一附属医院(以下简称"我院")多西他赛致药品不良反应(adverse drug reaction,ADR)的发生规律及特点。方法:采用回顾性研究方法,对2015年1月1日至2017年8月31日我院上报的61例多西他赛致ADR报告进行统计分析。结果:61例使用多西他赛发生ADR的患者中,绝大多数为>40岁者(58例,占95.08%);女性患者居多(35例,占57.38%);原患疾病以乳腺癌(17例,占27.87%)、肺癌(11例,占18.03%)、食道癌(11例,占18.03%)及妇科肿瘤(包括宫颈癌、卵巢癌和子宫内膜癌,共10例,占16.39%)为主;多西他赛致ADR的临床表现合计70例次,以胃肠道反应(31例次,占44.29%)、过敏反应(23例次,占32.86%)为主,骨髓抑制(2例次,占2.86%)较少。结论:临床应用多西他赛的过程中应采取相应的预防措施,做好用药监护,避免或减少严重的ADR发生,发生ADR后应及时予以处理。

OBJECTIVE： To investigate the general rules and characteristics of adverse drug reactions（ ADR）induced by docetaxel in the First Affiliated Hospital of Bengbu Medical College（ hereinafter referred to as ＂our hospital＂）. METHODS： Retrospective analysis method was adopted,and statistical analysis was conducted on 61 cases of ADR induced by docetaxel reported from our hospital from Jan. 1st,2015 to Aug. 31 st,2017. RESULTS：The majority of 61 patients who developed ADR induced by docetaxel were 40 years old（ 58 patients,95. 08%）;and female patients were the most（ 35 cases,57. 38%）. The original diseases were breast cancer（ 17 cases,27. 87%）,lung cancer（ 11 cases,18. 03%）,esophageal cancer（ 11 cases,18. 03%） and gynecological tumors（ including cervical cancer,ovarian cancer and endometrial cancer,totally 10 cases,accounting for 16. 39%）.Docetaxel-induced ADR had a total of 70 case-times clinical manifestation,including gastrointestinal reactions（ 31case-times,44. 29%）,allergic reactions（ 23 case-times,32. 86%）,and bone marrow suppression（ 2 case-times,2. 86%） accounted for only a small part. CONCLUSIONS： In the clinical application of docetaxel,appropriate preventive measures should be taken to monitor medications,so as to to avoid or reduce the occurrence of serious ADR; and it is necessary to dealt with it in time after the occurrence of ADR.
