加味八珍止眩汤治疗老年人颈性眩晕的临床疗效及对椎动脉血流量的影响

Clinical Curative Effect of Using Modified Bazhen Zhixuan Decoction in the Treatment of Elderly Cervical Vertigo and the Effect on BLOOD FLow of Vertebral Artery

目的:评价加味八珍止眩汤治疗老年人颈性眩晕的临床疗效以及对椎动脉血流量的影响。方法:选择2015年3月~2016年11月来我科室就诊的颈性眩晕患者140例,根据excel表格法分为两组,70例口服加味八珍止眩汤(中药组),70例口服西比灵胶囊(西药组),各治疗2周。根据颈性眩晕症状与功能评估量表对两组患者治疗前后进行评分,并于治疗前后给予TCD超声检查,测定基地动脉、两侧椎动脉的平均血流速度,并评定疗效。结果:研究结果显示,经过2周的治疗,中药组有效率为94. 29%,西药组有效率为84. 29%,中药组有效率显著高于西药组(P<0. 05);与治疗前相比,两组治疗后在眩晕、颈肩痛、头痛、日常生活及工作、心理及社会适应方面的评分均显著降低,组间比较结果显示,中药组治疗后各方面的评分显著低于西药组(P<0. 05);与治疗前相比,两组治疗后基地动脉、左侧椎动脉、右侧椎动脉收缩期的血流速度均显著升高,中药组收缩期的血流速度高于西药组(P<0. 05)。结论:加味八珍止眩汤治疗老年人颈性眩晕,可以有效的缓解相关症状,加快椎动脉血流速度,改善颈椎微循环,效果显著,值得临床推广。

Objective： To evaluate the clinical efficacy of using Bazhen Zhixuan Decoction in the treatment of elderly cervical vertigo and the effect on blood flow of vertebral artery. Methods： 140 patients with cervical vertigo from March 2015 to November2016 in our department from an example,according to the excel table were divided into two groups,70 cases of oral Bazhen Zhixuan Decoction（ Chinese medicine group）,70 cases of oral Sibelium Capsules（ western medicine group）,2 weeks of treatment.According to the evaluation of cervical vertigo symptoms and function scale before and after treatment in two groups were given TCD and ultrasonography before and after treatment,the average flow velocity of base artery,vertebral artery,and to evaluate the curative effect. Results： The results showed that after 2 weeks of treatment,the traditional Chinese medicine group,the effective number of 66 people,was 94. 29%,the western medicine group,the effective number of 59 people,efficiency of 84. 29%,the Chinese medicine group was significantly higher than that in the western medicine group（ P〈0. 05）; Compared with before treatment,the two groups after treatment in vertigo and the neck shoulder pain,headache,daily life and work,psychological and social adaptation of the scores were significantly lower,comparison between groups showed that the traditional Chinese medicine group after treatment the score was significantly lower than that in the western medicine group（ P〈0. 05）; Compared with before treatment,the two groups after treatment,arterial blood flow velocity of vertebral artery and the left base the right vertebral artery systolic blood flow velocity increased significantly,the traditional Chinese medicine group were higher than that of the western medicine group（ P〈0. 05）. Conclusion： Using Bazhen Zhixuan Decoction in the treatment of elderly cervical vertigo,can effectively relieve the symptoms,accelerate the blood velocity of vertebral artery,improve microcirculation of cervical vertebra,the effect is significant,worthy of promotion.
