栅栏状中性粒细胞肉芽肿性皮炎合并强直性脊柱炎一例

Palisade neutrophilic and granulomatous dermatitis complicated with ankylosing spondylitis: a case report

患者,男,55岁。四肢暗红色斑块、脱屑5年。既往有强直性脊柱炎病史。皮损组织病理学检查示:真皮内上皮细胞肉芽肿改变,部分区域上皮细胞呈栅栏状排列,局部伴有中性粒细胞、淋巴细胞浸润。实验室检查抗核抗体、SSA、SSB、抗双链DNA等阴性,HLA-B27阳性。诊断:栅栏状中性粒细胞肉芽肿性皮炎合并强直性脊柱炎。

A 55-year-old male presented with erythematous plaques and scales on the extensor surface of extremities for 5 years. He has a history of ankylosing spondylitis（ AS） for 10 years. The biopsy showed granulomatous nodules in the dermis,and some histiocytes,lymphocytes,neutrophils in a pattern of palisading arrangement. Antinuclear antibody,anti-SS-B,anti-SS-A and anti-double-stranded DNA were negative. HLA-B27 was positive. The diagnosis of palisaded neutrophilic granulomatous dermatitis companied with ankylosing spondylitis was made.
