综合性康复治疗对头颈部肿瘤患者放疗后情绪、体力及生活质量的影响

Effect of comprehensive rehabilitation on emotion, physical strength and quality of life for patients with head and neck cancer after radiotherapy

目的探讨综合性康复治疗对头颈部肿瘤患者放疗后情绪、体力及生活质量的影响。方法选取行根治性放射治疗的头颈部恶性肿瘤患者87例,根据随机数字表法将患者随机分为观察组(n=43)和对照组(n=44)。放疗后,对照组患者给予常规健康教育,观察组患者在常规健康教育的基础上给予包括心理疏导、体力训练和营养教育的综合性康复治疗。对两组患者康复治疗前后的焦虑抑郁评分、体力状况及生活质量进行比较。结果康复治疗前,两组患者的焦虑指数、抑郁评分,非利手握力,生活质量评分比较,差异均无统计学意义(P﹥0.05)。康复治疗后,观察组患者的焦虑和抑郁评分均明显低于本组康复治疗前,非利手握力和生活质量评分均明显高于本组康复治疗前,差异均有统计学意义(P﹤0.01)。康复治疗后,观察组患者的焦虑和抑郁评分均明显低于对照组(P﹤0.01),非利手握力和生活质量评分均高于对照组(P﹤0.05)。结论综合性康复治疗能够有效缓解头颈部肿瘤患者放疗后的焦虑和抑郁情绪,改善患者的体力状况,提高其生活质量。

Objective To explore the effect of comprehensive rehabilitation on emotion, physical strength and quality of life for patients with head and neck cancer after radiotherapy. Method Eighty-seven patients with head and neck malignant cancer after radical radiotherapy were selected and randomly divided into observation group（n=43） and control group（n=44）. The patients in control group were guided with routine health education after radiotherapy, and the patients in observation group were additionally administered with comprehensive rehabilitation, including psychological guidance, physical training and nutrition consultation. The anxiety/depression score, physical strength and quality of life at beginning and end of the rehabilitation treatment in two groups were analyzed and compared. Result There were no significant differences in the scores of the anxiety/depression score, physical strength and quality of life between two groups at the beginning of rehabilitation treatment（P〉0.05）. After rehabilitation treatment, the scores of the anxiety/depression were obvious decreased in both groups, and the non-dominant hand grip strength and scores of quality of life were significantly increased in both group（P〈0.01）. The scores of the anxiety/depression score in observation group at the end of rehabilitation treatment were significantly lower than those in control group（P〈0.01）, while the scores of physical strength and quality of life in observation group at the end of rehabilitation treatment were significantly higher than those in control group（P〈0.05）. Conclusion Comprehensive rehabilitation can effectively relieve the anxiety/depression, improve physical strength and quality of life of patients with head and neck cancer after radiotherapy.
