摘要
目的探讨细胞间黏附分子-1(ICAM-1)、基质金属蛋白酶-9(MMP-9)及上皮细胞钙黏蛋白(E-cadherin)在结直肠癌中的表达及意义。方法采用免疫组化法检测68例结直肠癌组织及68例癌旁正常组织中ICAM-1、MMP-9及E-cadherin的表达,并分析三者与临床特征的关系。结果 ICAM-1、MMP-9在结直肠癌组织中的阳性表达率明显高于癌旁正常组织,E-cadherin在结直肠癌组织中的阳性表达率明显低于癌旁正常组织,差异均有统计学意义(P﹤0.01)。C+D期、有淋巴结转移及低分化的结直肠癌患者ICAM-1、MMP-9阳性表达率均明显高于A+B期、无淋巴结转移及高中分化的结直肠癌患者,差异均有统计学意义(P﹤0.01);C+D期、有淋巴结转移及低分化的结直肠癌患者E-cadherin阳性表达率均明显低于A+B期、无淋巴结转移及高中分化的结直肠癌患者,差异均有统计学意义(P﹤0.01)。不同年龄、性别、肿瘤大小、肿瘤部位结直肠癌组织中ICAM-1、MMP-9及E-cadherin阳性表达率比较,差异均无统计学意义(P﹥0.05)。结直肠癌中ICAM-1与MMP-9表达呈显著正相关,ICAM-1与E-cadherin表达呈显著负相关,MMP-9与E-cadherin表达呈显著负相关。结论高表达的ICAM-1、MMP-9及低表达的E-cadherin对结直肠癌的发生、浸润、转移起促进作用,三者联合检测可能对结直肠癌的诊断起指导作用。
Objective To explore the significance and expression of intercellular cell adhesion molecule-1(ICAM-1),matrix metalloproteinase-9(MMP-9) and E-cadherin in colorectal cancer. Method The expression of ICAM-1, MMP-9 and E-cadherin in 68 cases of colorectal cancer and adjacent normal tissues were detected immunohistochemically. The relationship between ICAM-1, MMP-9, E-cadherin and clinical characterisitics was analyzed. Result The positive rate of ICAM-1, MMP-9 in colorectal cancer tissues was higher than that in adjacent normal tissues(P〈0.01). The positive rate of E-cadherin in colorectal cancer tissues was lower than that in adjacent normal tissues(P〈0.01). Higher positive rate of ICAM-1 and MMP-9, lower positive rate of E-cadherin were observed in patients with stage C+D disease, with lymph node metastases, and poor differentiation compared to those with stage A+B, no lymph node metastases, or with high or moderate differentiations, showing statistically significant difference(P〈0.01). Age, gender, tumor size and tumor location were not associated with the positive rates of ICAM-1, MMP-9 and E-cadherin(P〉0.05). The expression of ICAM-1 was positively correlated with the expression of MMP-9 in colorectal cancer, while was negatively correlated with the expression of E-cadherin, and so did for MMP-9. Conclusion High expression of ICAM-1 and MMP-9 as well as low expression of E-cadherin plays an important role in the occurrence, invasion and metastasis of colorectal cancer. The combined detection of these three proteins is helpful for the diagnosis of colorectal cancer.
