阿法骨化醇胶丸与碳酸钙D_3片联合唑来膦酸钠治疗53例老年骨质疏松症患者的短期随访研究

Alfacalcidol Soft Capsules and Calcium Carbonate D3 Tablets Combined with Zoledronic Acid in the Treatment of 53 Elderly Patients with Osteoporosis: a Short-term Follow-up Study

目的:研究老年骨质疏松症患者以阿法骨化醇胶丸与碳酸钙D_3片联合唑来膦酸钠治疗的临床效果。方法:选取2015年1月～2017年10月济南军区总医院老年骨质疏松症患者105例,按随机数字表法分观察组(n=53)、对照组(n=52)。对照组以碳酸钙D3片、唑来膦酸钠配合治疗,观察组以对照组为基础进一步加用阿法骨化醇胶丸治疗。对比两组疗效、治疗前及治疗6个月后骨钙素(BGP)、疼痛程度(VAS评分)、骨密度(BMD)、生活质量(SF-36评分)、不良反应。结果:观察组总有效率为96.23%(51/53),高于对照组80.77%(42/52,P<0.05);治疗6个月后观察组BGP、BMD大于对照组(P<0.05);治疗6个月后观察组VAS评分较对照组低,SF-36评分较对照组高(P<0.05);观察组不良反应发生率0.00%(0/49)与对照组8.33%(4/48)对比无显著差异(P>0.05)。结论:老年骨质疏松症患者以阿法骨化醇胶丸与碳酸钙D3片联合唑来膦酸钠治疗临床效果显著,能有效改善骨代谢,缓解疼痛,提高生活质量,且不良反应发生率低。

Objective To explore the clinical effect of Alfacalcidol Soft Capsules and calcium carbonate D3 tablets combined with zoledronic acid in the treatment of senile osteoporosis patients. Methods： 105 cases of senile osteoporosis in our hospital from January 2015 to October 2017 were selected and divided into observation group （n=53） and control group （n=52） according to random number table. The control group was treated with calcium carbonate D3 tablets and zoledronic acid. The observation group was further treated with Alfacalcidol Soft Capsules on the basis of the control group. The efficacy, 0steocalcin （BGP）, pain （VAS score）, bone mineral density （BMD） and quality of life （SF-36 score） before and 6 months after treatment and adverse reaction were compared between the two groups. Results： The total effective rate of the observation group was 96.23% （51/53）, which was higher than that of the control group （80.77%） （42/52）（P〈0.05）. After 6 months of treatment, the BGP and BMD in the observation group were greater than those in the control group（P〈0.05）. After treatment for 6 months, the VAS score of the observation group was lower than that of the control group, and the SF-36 score was higher than that of the control group（P〈0.05）. The incidence of adverse reactions in the observation group was 0% （0/49）, and there was no difference between the observation group and the control group 8.33% （4/48）（P〉0.05）. Conclusion： Alfacalcidol Soft Capsules and calcium carbonate D3 tablets combined with zoledronic acid in the treatment of senile osteoporosis patients has significant clinical effect. It can effectively improve bone metabolism, relieve pain, improve the quality of life, and the incidence of adverse reactions is low.
