评估踝臂指数在下肢动脉硬化闭塞症介入治疗中的临床价值

Evaluation of the clinical value of ankle brachial index(ABI) in interventional therapy of lower extremity arteriosclerosis obliterans

目的分析下肢动脉硬化闭塞症(LEAOD)介入治疗中踝臂指数(ABI)的临床应用效果。方法 2015年12月～2017年11月,于我院收治的下肢动脉硬化闭塞症患者中选取43例,所有患者均得到确诊,为其制定针对性的介入治疗方案,术后对其踝臂指数(ABI)进行测量,分析下肢动脉硬化闭塞症介入治疗中踝臂指数的临床应用效果。结果踝臂指数越低,患者的血管病变严重程度与临床症状越重,P <0.05;当患者踝臂指数大于0.90时,均没有多支血管阻塞、双支血管阻塞或者严重肢体缺血现象出现;进行介入治疗之后,患者患肢血流运行情况在很大程度上好转,或者血流处于畅通状态,间歇性跛行、静息痛等症状得到明显改善或者完全消失,有例患者合并有脚趾溃疡,经过治疗后,其溃疡面积在很大程度上缩小;患者介入治疗前后的收缩压、PWV、ABI进行比较,P <0.01。结论踝臂指数具有快捷、无创等优点,在下肢动脉硬化闭塞症介入治疗前后具有良好的应用价值。

Objective To analyze of clinical application of ankle braehial index （ABI） in interventional treatment of lower extremity arteriosclerosis obliterans（LEAOD）. Methods From December 2015 to November 2017, 43 eases of lower extremity arteriosclerosis obliterans admitted in our hospital were selected. All the patients were diagnosed as definite interventional treatment plan. After the operation, Index （ABI） was measured to analyze the clinical effect of ankle braehial index in the interventional treatment of lower extremity arterial occlusive disease. Results The lower the ankle-braehial index, the more severe the vascular lesions and clinical symptoms of patients, P 〈 0.05. No multi-vessel obstruction, double-vessel obstruction or severe limb isehemia were observed when the ankle-braehial index was greater than 0.90 Appeared; after interventional treatment, the patient＇s limb blood flow was greatly improved, or the blood flow was unobstructed, intermittent elaudieation, rest pain and other symptoms have been significantly improved or completely disappeared, and some patients with toe Ulcer, after treatment, the ulcer area in a large extent reduced; patients before and after interventional treatment of systolic blood pressure, PWV, ABI were compared, P 〈 0.01. Conclusion The ankle braehial index has the advantages of quickness and non-invasiveness, and has good application value before and after interventional treatment of lower extremity arteriosclerosis obliterans.
