摘要
目的分析湖州市2011年-2015年妇科门诊人乳头状瘤病毒(human papilloma virus,HPV)感染状况,亚型分布及其与年龄的相关性,为HPV感染预防、疫苗选择及研发提供参考依据。方法回顾性分析2011年1月-2015年12月湖州市中心医院妇科门诊患者宫颈脱落细胞HPV基因分型结果。结果在24 446份标本中检出HPV阳性6 375例,总感染率为26. 08%(6 375/24 446);前3位感染亚型为HPV52、HPV16、HPV58;其构成比分别为19. 85%(1 713/8 630)、15. 09%(1 302/8 630)、11. 30%(975/8 630)。高危感染占85. 24%(5 434/6 375),低危感染占7. 44%(474/6 375),高低危混合感染占7. 33%(467/6 375),此三者在<20岁年龄组均最高(33. 93%、10. 71%、15. 48%)。HPV总体感染以单一亚型感染为主(72. 78%,4 640/6 375),45岁~年龄组感染率最高(77. 63%),多重亚型感染率最高的是<20岁年龄组(55. 45%)。结论湖州市妇科门诊患者HPV感染率为26. 08%,亚型以HPV52、HPV16、HPV58为主。感染率年龄分布呈U型,<20岁和≥55岁人群HPV感染率较高,未来疫苗的选择及研发应考虑HPV亚型的地区流行特征。
Objective To analyze the human papilloma virus(HPV) infection in Huzhou, Zhejiang Province from 2011 to 2015 and the correlation between subtype distribution and age, so as to provide reliable reference for HPV infection prevention, vaccine selection and research. Methods To retrospectively analyze the HPV genotyping results of cervical exfoliated cells from gynecological outpatients in Huzhou Central Hospital from January 2011 to December 2015. Results Of the 24 446 specimens, 6 375 were positive ( 26.08% ), the HPV genotype detection indicated that the top 3 HPV types were HPV52 ( 19.85%, 1 713/8 630) , HPV16( 15.09% , 1 302/8 630) and HPV58 (11.30%, 975/8 630). The high -risk infection accounted for 85.24% (5 434/6 375 ), the low risk infection accounted for 7.44% (474/6 375 ), the high/low risk co - infection accounted for 7.33% (467/6 375), which reached the high^st in age group of less than 20 years old(33.93% , 10.71% , 15.48% ). Single subtype infection was the dominant in HPV infections (72.78% , 4 640/6 375 ), the highest infection rate was observed in the 45 -49 age group (77.63%), and the multi - subtype infection rate was the highest in the group of less than 20 years old (55.45%). Conclusion The HPV infection rate was 26.08% among the gynecology outpatients in Huzhou. The HPV type is dominated by HPV52, HPV16 and HPV58. The infection rate is U - shaped, HPV infection rate is greater in people younger than 20 and over 55 years old. The selection and development of future vaccines should consider the regional epidemic features of HPV subtype.
