血清乳酸含量对肺炎病情的判断价值及其与机体细胞免疫功能的相关关系

Value of serum lactic acid in pneumonia diagnosis and its correlation with cellular immune function

目的探讨血清乳酸含量对肺炎病情的判断价值及其与机体细胞免疫功能的相关关系。方法收集2016年4月-2017年6月本院收治的成人肺炎患者117例,根据肺炎病情严重程度将观察组患者分为普通肺炎组79例、重症肺炎组38例,另取同期在本院进行体检的健康志愿者50例作为正常对照组。测定血清中乳酸、Th1/Th2及Th17/Treg细胞因子的含量。结果重症肺炎组、普通肺炎组患者血清中乳酸、白细胞介素-4 (IL-4)、白细胞介素-17(IL-17)、IL-17/IL-10的水平高于正常对照组,干扰素-γ(IFN-γ)、IFN-γ/IL-4、IL-10的水平低于正常对照组;重症肺炎组血清中上述指标的变化较普通肺炎组更为显著,差异均有统计学意义(P <0. 05)。结论随肺炎病情加重,患者血清中乳酸含量增加,且具体乳酸含量可客观反映机体细胞免疫功能。

Objective To study the value of serum lactic acid in pneumonia diagnosis and its correlation with cellular immune function. Methods A total of 117 pneumonia patients collected in our hospital during April 2016 - June 2017 were divided into ordinary pneumonia group（ n = 79） and severe pneumonia group （n = 38 ） according to the severity of pneumonia, and another 50 healthy volunteers undergoing medical examination in our hospital were chosen as normal control group. Serum contents of lactic acid, Th1/Th2 and Th17/Treg cytokines were detected. Results The levels of serum lactic acid, IL -4, IL - 17, IL - 17/lL - 10 in severe pneumonia group and ordinary pneumonia group were higher than that in normal control group, while the levels of serum IFN-γ, IFN-/IL- 4, IL- 10 were lower than those in normal control group; the changes of the above indexes in the severe pneumonia group were more significant than those in the common pneumonia group, with the differences statistical significance （ P 〈 0.05 ）. Conchlsion As the exacerbation of pneumonia, serum content of lactic acid increases, and the specific lactic acid content can objectively reflect the cellular immune function.
