摘要
目的研究SLE患者不同活动度中IgG亚类分布特点和临床意义。方法检测不同活动度SLE患者和健康人群血清中IgG亚类水平,结合患者临床资料计算SLE活动度指数(SLEDAI),按SLEDAI分组比较分析检测结果。结果相对于健康对照组,SLE轻度活动患者IgG水平增高,IgG1、IgG3增高(t=1. 693,P <0. 05),IgG2、IgG4下降,差异有统计学意义(t=-2. 692、t=-6. 046,P <0. 05)。SLE中度活动患者IgG增高(t=2. 335,P <0. 05水平),IgG1、IgG3增高,差异有统计学意义(t=2. 423、t=1. 823,P <0. 05),IgG2、IgG4下降。SLE重度活动患者IgG水平增高(t=1. 598,P <0. 05),IgG1、IgG3增高,差异有统计学意义(t=2. 241、2. 587,P <0. 05),IgG2(t=-1. 100,P <0. 05,)、IgG4下降。结论 SLE患者不同活动度中IgG亚类分布水平较健康人群有明显差异,IgG1升高、IgG4下降明显; IgG4可用于鉴别IgG4相关性疾病;并可用于病情变化的监测指标。
Objective To study the distribution and clinical signifieance of IgG subclass in SLE patients with different activity. Methods Serum IgG subtypes in SLE patients and healthy subjects with different activities were detected. Disease activity index (SLEDAI) in SLE patients was calculated according to clinical data, and the results were compared according to SLEDAI group. Results Compared with the control group, the IgG level of SLE patients with mild activity increased, IgG1 and IgG3 inereased, and IgG2 and IgG4 decreased, with the differences statistically significant ( t = - 2.692, t = - 6. 046, P 〈 0.05 ). The IgG level in patients with moderate activity increased, IgG1 and IgG3 inereased, with the differences statistically significant ( t = 2. 423, t = 1. 823, P 〈 0.05 ), and IgG2 and IgG4 decreased in SLE. The IgG level of patients with severe SLE activity increased, IgG1 and IgG3 inereased(t = 2. 241, t = 2. 587, P 〈 0.05), and IgG2 ( t = - 1. 100, P 〈 0.05 ), IgG4 decreased. Conclusion The distribution level of IgG subtypes in SLE patients is significantly different from that in healthy people. IgG1 increased and IgG4 decreased significantly. IgG4 can be used to identify IgG4 related diseases, and can be used for monitoring the indicators of disease changes.
