关节超声对痛风关节炎急性发作期的诊断价值 被引量:9

摘要 目的探讨关节超声在痛风关节炎急性发作期的诊断价值。方法选取2017年1月至2017年12月急性痛风性关节炎患者30例为实验组,同期选取30例急性非痛风性关节炎患者为对照组,两组患者均采用关节超声检查。比较两组患者的超声表现特点,观察双轨征阳性患者的不同关节区域超声表现。结果实验组患者的双轨征、关节内或周围结节超声表现例数多于对照组,差异有统计学意义(P <0. 05);在所有扫查项目中,实验组与对照组患者关节内或关节周围结节、双轨征超声影像数据比较,差异有统计学意义(P <0. 05)。足趾关节与膝关节发生双轨征的阳性率高于其他部位,差异有统计学意义(P <0. 05)。结论关节超声在痛风关节炎急性发作期诊断中的应用效果显著,能够有效鉴别患者的疾病状况,尤其是借助双轨征可以准确判断患者是否属于痛风关节炎,其中膝关节与足趾关节属于重点筛查区域。 Objective To investigate the diagnostic value of joint ultrasound in acute attack period of gouty arthritis. Methods Thirty patients with acute gouty arthritis from January 2017 to December 2017 were selected as the experimental group, and 30 patients with acute non gouty arthritis were selected as the control group at the same period. All of the patients in the two groups were examined by joint ultra- sonography. The uhrasonographic features and the ultrasonic manifestations of different joint regions in patients with double track signs were compared between the two groups. The ultrasonographic characteris- tics of the two groups were compared, and the ultrasonic manifestations of the different joint regions were observed. Results The number of ultrasonic manifestations in the experimental group was more than that of the control group, and the difference was significant ( P 〈 0.05 ). There was significant difference in the imaging findings of nodules and double tracks around the joints or joints between the experimental group and the control group (P 〈 0.05 ). The positive rate of ultrasonic and imaging data of nodules and double tracks around the joint or in the joints was higher than that of other parts, and the difference was significant (P 〈 0.05 ). Conclusions The application of joint ultrasound in the diagnosis of acute gout arthritis is significant. It can effectively identify the patient's condition, especially the double track sign can accurately determine whether the patient belongs to gout arthritis, and the knee and toe joint belong to the key screening area.
