基于动机激发策略下快速康复外科理念对肾癌患者腹腔镜根治术镇痛与依从性的影响

Effect of rapid rehabilitation surgery based on motivational stimulation strategy on analgesia and compliance of laparoscopic radical nephrectomy for patients with renal cell carcinoma

目的探讨基于动机激发策略下快速康复外科理念对肾癌腹腔镜根治术镇痛及依从性的影响。方法选取我院2013年12月至2017年12月收治的择期腹腔镜肾癌根治术患者102例,随机将其等分为研究组和对照组,对照组实施常规护理,研究组实施动机激发策略下快速康复外科理念护理模式,比较两组患者疼痛程度和依从性。结果研究组术后1,3,7 d疼痛评分低于对照组(P <0. 05);研究组营养支持、镇痛治疗、早期进食、早期活动依从率高于对照组(P <0. 05);研究组患者术后通气时间、进食时间、下床时间及住院时间均短于对照组(P <0. 05)。结论基于动机激发策略下快速康复外科理念可有效控制腹腔镜肾癌根治术患者术后镇痛效果,提高患者主动配合治疗依从性,提高临床预后效果。

Objective To explore the effect of rapid rehabilitation surgery based on motivational stimulation strategy on analgesia and compliance of laparoscopic radical nephrectomy for patients with renal cell carcinoma. Methods To select a total of 102 cases of patients with elective laparoscopic radical nephrectomy admitted to our hospital from December 2013 to December 2017, and they were divided into study group and control group randomly. The control group was given routine nursing, while the study group was implemented with rapid rehabilitation surgical concept nursing model based on motivational stimulation strategy. The pain level and compliance of patients in the two groups were compared. Results The pain scores of the study group were lowerthan that of the control group at 1, 3, and 7 days after operation（P 〈 0. 05）. The nutritional support, analgesic treatment, early feeding, and early activity compliance rate of the study group were higher than those of the control group（P 〈 0. 05）. The postoperative ventilation time, eating time, out - of - bedtime and hospitalization time of patients in the study group were all shorter than that of the control group（P 〈 0. 05）. Conclusion The concept of rapid rehabilitation surgery based on motivational stimulation strategy can control the postoperative analgesia effect of patients undergoing laparoscopic radical nephrectomy effectively. In addition, it can improve both the patient＇ s active compliance with treatment and the clinical prognosis.
