The diagnosis of MSCT for the pleomorphic adenomas in palatine

摘要 目的探讨腭部多形性腺瘤的CT影像学表现,旨在提高该病诊断准确率。方法回顾性分析本院经手术病理证实的21例腭部多形性腺瘤患者的临床表现及CT影像学特征。所有患者均行腭部CT平扫,其中有18例患者行增强CT扫描,重点观察病灶所在部位、CT值、密度及与周围组织的关系。结果 21例病变中发生于一侧硬腭8例,软腭7例,软硬腭交界处6例;平均最大径约2. 6cm; 15例呈圆形或类圆形,6例呈分叶状,20例病灶边界均较清楚,1例边界不清。肿瘤多呈软组织密度,低于或等于邻近肌肉密度,12例密度较均匀,9例密度不均匀,7例病灶内见坏死囊变区,2例病灶内见钙化灶。CT增强检查示病变实性部分呈轻至中度持续强化。结论腭部多形性腺瘤的CT平扫及增强扫描具有一定特征性,结合病史可以做出定性诊断。 Objective To explore the CT imaging features of the pleomorphie adenomas in palatine, in order to improve the diagnostic accuracy of the disease. Methods CT imaging manifestations and Clinical data of 21 patients with palatal polymorphie adenomas were analyzed retrospectively, all of them were examined by plain CT scan, and in 11 patients, additional contrast-en- hanced scanning was carried out. The location, CT value, density and the relationship with the surrounding tissue were observed. Results Among 21 cases, 8 tumors located in hard palate, 7 in soft palate, and 6 tumors in the junction of hard and soft; the average maxinmnl diameter was about 2.6 cm; 15 eases were round or oval, and 6 eases were lobulated; the boundary of the lesions in 20 eases was clear, only 1 ease had unclear boundary and was considered to be caused by infection. The tumor was mostly soft tissue density, lower than or equal to adjacent muscle density. The density of 12 eases was relatively uniform, the density of 9 eases was not uniform, and 7 eases showed the patchy low-density necrotic cystic region, and the calcification range was seen in 2 eases. CT enhancement examination showed that the solid part of the lesion was light to moderate continuous enhancement. Conclusion CT scan and enhanced scan of palatal pleomorphie adenoma have some characteristics, which can be diagnosed qualitatively according to their medical history.
