摘要
目的探讨联合超声多模态产前诊断胎盘植入的临床应用价值。方法选取2014年11月至2017年12月在芝罘区妇幼保健院临床检查前置胎盘的患者215例,产前均行高频超声、低频超声及三维能量多普勒成像检查,依据产时临床诊断和产后病理检查结果,分析低频超声、高、低频超声联合及多模态超声(高、低频超声及三维能量多普勒)联合诊断胎盘植入的敏感度、特异度、准确度、阳性预测值、阴性预测值。结果 215例患者中胎盘植入共39例。多模态超声技术诊断胎盘植入的敏感度、特异度、准确度、阳性预测值、阴性预测值(84.6%、97.8%、95.3%、82.9%、96.6%)均高于低频超声(66.7%、94.3%、89.3%、72.2%、92.7%)和高、低频超声联合(74.4%、96.0%、92.1%、80.6%、94.4%)。结论多模态超声技术联合可以提高对胎盘植入的诊断效能,降低漏诊率,具有广阔的应用前景和临床价值。
Objective To explore the differential diagnosis application value of multinlodal ultrasound technologies in placenta accretas. Methods Two hundred and fifteen cases of placenta prevail patients underwent combined nmltiple ultrasound inspection from November 2014 to December 2017 in our hospital. Based on the clinical diagnosis and postpartunl pathological examination results, the sensitivity-, specificity-, accuracy, positive predictive value and negative predictive value of ultrasound, low-frequency ultrasound combined with high-frequency ultrasound and multinlodal ultrasound technologies (low-frequency ultrasound, high-frequency ultrasound combined with three-dimensional power Doppler ) were calculated. Results In the 215 cases, 39 cases were placenta accreta. Generally, the specificity-, sensitivity-, accuracy, positive predictive value and negative predictive value of the combined multimodal ultrasound technologies (84.6%, 97.8%, 95.3%, 82.9%, 96.6%)were significantly higher than abdominal(66.7%, 94.3%, 89.3%, 72.2%, 92.7%), low-frequency ultrasound combined with high-freuency (74.4%, 96.0%, 92.1%, 80.6%, 94.4%). Conclusion The combination of multimodal ultrasound technologies can improve the diagnostic efficiency of placenta accreta and reduce the missed diagnosis rate. It has broad application prospects and clinical value.
