Nd:YAG激光联合改良型器械的龈下刮治和根面平整术治疗老年人Ⅱ度根分叉病变的临床效果评价

Clinical evaluation of Nd:YAG laser combined with modified instruments for scaling and root planing in the treatment of Molar Class Ⅱ Furcation disease in the elderly

目的:探讨Nd:YAG激光辅助改良型器械SRP对老年人Ⅱ度根分叉病变的临床效果评估。方法:选取60～73岁以上牙周炎患者的56颗II度根分叉病变患牙,行超声龈上洁治术一周后,记录牙周临床指数,随机分为实验组:同颌牙一侧使用Nd:YAG激光牙周治疗仪辅助改良型器械SRP;对照组:对侧同名牙单纯使用改良型器械SRP治疗,3个月后评估各牙周临床指数。结果:实验组和对照组治疗后PLI、mBI、PD、均较治疗前有明显差异,P<0. 05。HPD、AL较治疗前有所减低,但差异均无统计学意义,P> 0. 05。PLI、PD在实验组和对照组在治疗后有较明显差异,P<0. 05。其余临床指标治疗前后两组间差异不大,P> 0. 05。结论:未发现Nd:YAG激光辅助SRP相比单纯SRP对于老年人Ⅱ度根分叉病变的治疗有明显优势,但可在一定程度上改善牙周袋深度和根面光洁度。

Objective： To evaluate the clinical effect of Nd：YAG laser-assisted modified instrument SRP in the treatment of Molar Class Ⅱ Furcation in the elderly. Methods： Fifty-six teeth with Molar Class Ⅱ Furcation disease Ⅱ were selected from 60 to 73 years old patients with periodontitis. The periodontal clinical index was recorded and randomly divided into two groups after ultrasonic supragingival scaling for one week. One group was treated with Nd：YAG laser periodontal therapy instrument and the other group was treated with modified instruments SRP. The clinical index of each periodontal ligament was evaluated 3 months after SRP treatment. Results： After treatment, PLI, m BI and PD in the experimental group and the control group were significantly different from those before treatment, P〈0.05. HPD and AL decreased after treatment, but the difference was not statistically significant, P〉0.05. PLI and PD were significantly different between the experimental group and the control group after treatment, P〈0.05. The other clinical indicators were not significantly different between the two groups before and after treatment, P〉0.05. Conclusion： Nd：YAG laser-assisted SRP has no obvious advantages in the treatment of type Ⅱ bifurcation lesions in the elderly, but it can improve the depth of periodontal pockets and root surface finish to some extent.
