摘要 目的分析不同剂量舒芬太尼对靶控异丙酚麻醉诱导血流动力学、脑电双频指数(BIS)的影响。方法 70例全身麻醉(全麻)手术患者,随机分为A组和B组,各35例。两组患者给予不同剂量的枸橼酸舒芬太尼注射液, A组为0.5μg/kg, B组为1μg/kg。观察两组患者麻醉诱导前、诱导后、插管后3 min及插管后10 min的心率(HR)、平均动脉压(MAP)、BIS,并进行比较。结果两组患者诱导后、插管后3 min及插管后10 min的BIS水平显著低于诱导前,差异具有统计学意义(P<0.05);插管后3 min,A组HR、MAP均高于诱导前,两组HR、MAP、BIS均高于诱导后,差异具有统计学意义(P<0.05);插管后10 min,两组HR、MAP、BIS均显著低于诱导前、诱导后及插管后3 min,差异具有统计学意义(P<0.05);诱导后、插管后3 min及插管后10 min, B组HR、MAP、BIS均低于A组,差异具有统计学意义(P<0.05)。结论给予全麻手术患者不同剂量的舒芬太尼,对其气管插管血流动力学以及BIS具有不同的影响,1μg/kg的舒芬太尼优于0.5μg/kg的舒芬太尼, BIS能够作为诱导过程中的镇静程度监测。 Objective To analyze the effects of different doses of Sufentanil on hemodynamics and bispectral index induced by target-controlled Propofol anesthesia. Methods A total of 70 patients with general anesthesia surgery were randomly divided into group A and group B, 35 cases in each group. Patients in the two groups were given different doses of Sufentanil Citrate Injection, group A was 0.5 g/kg, group B was 1 g/kg. The heart rate (HR), mean arterial pressure (MAP) and BIS before and after anesthesia induction, 3 min after intubation and 10 min after intubation were observed and compared. Results The levels of BIS in the two groups after induction, 3 min after intubation and 10 min after intubation were significantly lower than those before induction, the differences were statistically significant (P〈0.05). 3 min after intubation, the HR and MAP in group A were significantly higher than those before induction, and the HR, MAP and BIS in the two groups were significantly higher than those after induction, the differences were statistically significant (P〈0.05). 10 min after intubation,the HR, MAP and BIS in the two groups were significantly lower than those before intubation, after intubation and 3 min after intubation, with statistically significant differences (P〈0.05). The HR, MAP and BIS in group B were significantly lower than those in group A after induction, 3 min after intubation and 10 min after intubation (P〈0.05). Conclusion Different doses of Sufentanil have different effects on the intubation hemodynamics and BIS of patients with general anesthesia. 1 μg/kg Sufentanil is better than 0.5 μg/kg Sufentanil. BIS can be used to monitor the degree of sedation during induction.
