ERCP术治疗胆源性胰腺炎的综合效果及对AMYL、TBIL、ALT、CRP的影响

Comprehensive effect of ERCP in the treatment of biliary pancreatitis and its influence on AMYL,TBIL,ALT and CRP

目的研究内镜逆行胰胆管造影(ERCP)术治疗胆源性胰腺炎的综合效果及对血淀粉酶(AMYL)、总胆红素(TBIL)、谷丙转氨酶(ALT)、C-反应蛋白(CRP)的影响。方法将93例胆源性胰腺炎患者随机分为对照组(46例)和观察组(47例),对照组行胆总管切开取石T管引流术,观察组行ERCP取石术。比较两组的治疗效果。结果观察组腹痛缓解时间及住院时间明显短于对照组,住院费用明显低于对照组(P<0.05)。术后1、4、7 d,两组AMYL、TBIL及ALT水平均明显低于术前1 d(P<0.05);术后1 d,观察组的AMYL水平明显低于对照组(P<0.05);术后4、7 d,观察组AMYL、TBIL、ALT及CRP水平均明显低于同一时间对照组(P<0.05)。结论 ERCP取石术治疗胆源性胰腺炎临床效果显著,患者肝胆、胰腺等功能恢复较好。

Objective To study the comprehensive effect of endoscopic retrograde cholangiopancreatography （ERCP） in the treatment of biliary pancreatitis and its influence on serum amylase （AMYL）, total bilirubin （TBIL）, alanine aminotransferase（ALT）, C-reactive protein （CRP）. Methods Ninety-three patients with biliary pancreatitis were randomly divided into control group （46 cases） and observation group （47 cases）. The control group underwent choledocholithotomy with T-tube drainage,the observation group underwent ERCP lithotomy. The therapeutic effects of the two groups were compared. Results The remission time of abdominal pain and hospitalization time in the observation group were significantly shorter than those in the control group, and the hospitalization expense was significantly lower than that in the control group （P〈0.05）. At 1, 4 and 7 d after operation, the levels of AMYL, TBIL and ALT in the two groups were significantly lower than those at 1 d before operation（P〈0.05）. At 1 d after operation, the level of AMYL in the observation group was significantly lower than that in the control group（P〈0.05）. The levels of AMYL, TBIL, ALT and CRP in the observation group were significantly lower than those in the control group at 4 and7d after operation （P〈0.05）. Conclusion The ERCP lithotomy on biliary pancreatitis has significant effect, with high safety. The function of liver, gallbladder and pancreas recover well.
