无创正压机械通气在急性中毒合并呼吸衰竭治疗中的应用效果

摘要 目的探讨无创正压机械通气在急性中毒合并呼吸衰竭治疗中的应用效果。方法选取2016年3月至2018年3月我院184例急性中毒合并呼吸衰竭患者为研究对象,根据治疗方法将受试者分为对照组和研究组,每组92例,两组均实施常规的内科治疗,研究组在此基础上实施无创正压机械通气治疗。比较治疗前、后各临床指标变化及预后情况。结果治疗12 h后,两组患者PaCO2、MAP、HR、呼吸频率较治疗前均显著降低,PaO_2、ChE较治疗前显著升高(P<0.05),血pH值比较无统计学差异(P>0.05)。研究组患者治愈率显著高于对照组,气管插管率、病死率及并发症发生率均显著低于对照组(P<0.05)。结论无创正压机械通气有助于改善患者的气血指标及呼吸功能,减少并发症发生率,提高治愈率,在急性中毒合并呼吸衰竭治疗中具有较高的应用价值。 Objective To explore application effect of noninvasive positive pressure ventilation in the treatment of patients with acute poisoning and respiratory failure. Methods A total of 184 patients with acute poisoning and respiratory failure from March 2016 to March 2018 were selected as the subjects. According to the treatment methods, all patients were divided into control group and study group, with 92 cases in each group. The control group was treated with conventional medical treatment and the study group was treated with noninvasive positive pressure ventilation on the basis of the control group. The changes of clinical indexes and prognosis before and after treatment were compared. Results After 12 hours of treatment, the levels of PaCO2, MAP and HR and respiratory rate was significantly decreased compared to before treatment, and the levels of PaO2 and ChE significantly increased (P〈0.05), there was no significant difference in blood pH between the two groups (P〉0.05). The cure rate of the study group was significantly higher than that of the control group, and the tracheal intubation rate, mortality rate, and the incidence of complications in the study were significantly lower than those of the control group (P〈0.05). Conclusion Noninvasive positive pressure ventilation can improve blood indexes and respiratory functions, reduce the incidence of complications and improve the cure rate, which has a high application value in the treatment of acute poisoning with respiratory failure.
