术前鸦胆子油乳+常规化疗药物腹腔灌注化疗对手术切除胃癌病灶内癌基因表达的影响

Effect of preoperative Brucea javanica oil emulsion+conventional chemotherapeutic drugs intraperitoneal chemotherapy on the expression of oncogene in advanced gastric cancer after surgical resection

目的:研究术前鸦胆子油乳+常规化疗药物腹腔灌注化疗对手术切除胃癌病灶内癌基因表达的影响。方法:选择在我院诊断为进展期胃癌的患者,随机分为术前接受鸦胆子油乳+常规化疗药物腹腔灌注化疗的联合组、接受常规化疗药物腹腔灌注化疗的对照组。化疗前后测定血清中肿瘤标志物的含量,手术后测定病灶内癌基因的表达量。结果:化疗后,两组血清中CEA、CA199、G17、TK1的含量均呈显著降低趋势,且联合组血清中CEA、CA199、G17、TK1含量的降低趋势较对照组更为显著;手术后,联合组手术切除的胃癌病灶内CyclinD1、Fra-1、Bmi-1、c-Met、HER-2、CTS-D、MMP2、MMP7的mRNA表达量均显著低于对照组,PTEN、p16、KISS-1、Caspase-3、TSPYL-5、TIMP1、RECK的mRNA表达量均显著高于对照组。结论:术前鸦胆子油乳+常规化疗药物腹腔灌注化疗能够较单用常规化疗药物腹腔灌注化疗更为显著的调节胃癌病灶内癌基因的表达。

Objective： To study the effect of preoperative Brucea javanica oil emulsion ＋ conventional chemotherapeutic drugs intraperitoneal chcmotherapy on the expression of oncogene in advanced gastric cancer after surgical resection. Methods： The patients who were diagnosed with advanced gastric cancer in our hospital during the period of March 2015 to October 2017 were randomly divided into the combined group who received preoperative Brucea javanica oil emulsion ＋ conventional chemo therapeutic drugs intrapcritoneal chemotherapy, and control group received conventional chemotherapeutic drugs intraperitoneal chemotherapy. The tumor markers in serum were determined before and after chemothcrapy, and the expression level of oncogenes in the lesions was measured after operation. Results： After chemotherapy, the levels of CEA, CA199, G17 and TK1 in the serum of the two groups wcrc significantly decreased and the decreasing trend of CEA, CA199, G17 and TK1 in the serum of the combined group was more significant than that of the control group. The mRNA expressions of CyclinD1 , FRA-1, Bmi-, c-Met, HER-2, CAT-D, MMP2, MMP7 in gastric cancer of combined group wcrc significantly lower than the control group, the mRNA expressions of PTEN, p16, KISS-1, Caspasc-3, TSPYL- 5, TIMP1, RECK wcrc significantly higher than that of control group. Conclusion： Preoperative Brucca javanica oil emulsion ＋ conventional chemothcrapcutic drugs intrapcr itoneal chemotherapy can more significantly regulate the expression of oncogene in gastric cancer than conventional chemothera py drugs.
