牙石残留率对牙周治疗效果影响的研究

Effect of residual rate of calculus on periodontal treatment results

目的利用牙周内窥镜的可视化辅助,观察传统刮治和根面平整术后牙石残留情况,分析不同的牙石残留率对牙周治疗效果的影响。方法选取31位牙周炎患者的不同探诊深度患牙根面作为试验对象。洁治后进行龈下刮治及根面平整术,牙周内窥镜下检查牙周袋内残留牙石,采集、分析图像,计算牙石残留率。以龈下刮治前为基线,记录基线及3个月复诊时牙周临床指标,分析牙石残留率与牙周治疗效果的关系。结果龈下刮治后牙石残留率≤4. 60%时,90%牙面3个月后PD≤3mm;牙石残留率在4. 60%～12. 37%之间时,90%牙面3个月后PD=4～6mm;牙石残留率> 12. 37%时,90%牙面3个月后PD> 6mm。龈下刮治后牙石残留率与3个月复查时牙周探诊深度存在线性相关关系。结论牙石残留率影响牙周病的预后,龈下刮治及根面平整术配合患者良好的菌斑控制能获得较好的临床效果。

Objective To evaluate the effect of residual rate of calculus on periodontal treatment results. Methods A total of 31 periodontal patients received scaling and root planing. After treatment, the periodontal endoscope was used to examine the residual calculus and the images were collected and analyzed. The relationship between the residual rate of calculus and periodontal treatment effect was analyzed. Results When the residual rate of calculus was less than 4.60% , 90% of the probing depth was less than 3mm after three months. When the residual rate of calculus was 4.60% -12.37% , 90% of the the probing depth was 4-6mm. When the residual rate of calculus was more than 12.37% , the probing depth was mostly over 6mm. There was a linear correlation between residual calculus after subgingival scaling and root planing and periodontal probing depth after three months. Conclusion Residual rate of calculus could influence the prognosis of periodontal disease.
