内镜下金属钛夹与喷洒药物治疗上消化道出血的对照观察

Comparative Study on Endoscopic Titanium Clips and Drug Spraying in the Treatment of Upper Gastrointestinal Bleeding

目的比较内镜下金属钛夹与喷洒药物治疗上消化道出血的效果。方法选取我院收治的上消化道出血患者104例,随机分为两组各52例。对照组采用内镜下喷洒药物治疗,观察组采用内镜下金属钛夹治疗,比较两组的治疗效果。结果观察组的肠鸣音恢复正常时间、呕血消失时间、大便潜血转阴时间、引流管变清时间以及住院时间均显著短于对照组(均P <0.05)。观察组术后12 h、 24 h及48 h的止血成功率均显著高于对照组,术后7 d的再出血率显著低于对照组(均P <0.05)。结论内镜下金属钛夹治疗可有效缩短上消化道出血患者的症状消失时间,提高止血效果,降低再出血率。

Objective To compare the effects of endoscopic titanium clips and drug spraying in the treatment of upper gastrointestinal bleeding. Methods 104 cases of patients with upper gastrointestinal bleeding in our hospital were selected and randomly divided into two groups, with 52 cases in each group. The control group was treated with endoscopic drug spraying, and the observation group was treated with endoscopic titanium clips. The curative effects of the two groups were compared. Results The time for bowel sounds returning to normal, time for disappeared hematemesis, time for feces occult blood returning to negative, time for drainage fluid become clear, and hospitalization time of the observation group were significantly shorter than those of the control group （P〈0.05）. At 12 h, 24 h and 48 h after operation, the success rates ofhemostasis in the observation group were significantly higher than those in the control group （P〈0.05）; At 7 days after operation, the re-bleeding rate of the observation group was significantly lower than that of the control group （P 〈0.05）. Conclusions Eendoscopic titanium clips in the treatment of patients with upper gastrointestinal bleeding can effectively shorten the disappearing time of symptoms, improve the hemostasis effect, and reduce the re-bleeding rate.
