股骨近端防旋髓内钉内固定治疗老年骨质疏松性股骨粗隆间骨折的效果分析

Analysis on the Effect of Proximal Femoral Anti-Rotation Intramedullary Nail for Internal Fixation in the Treatment of Senile Osteoporotic Femoral Intertrochanteric Fracture

目的分析股骨近端防旋髓内钉内固定治疗老年骨质疏松性股骨粗隆间骨折的效果。方法选取我院2016年1月至2017年11月收治的116例老年骨质疏松性股骨粗隆间骨折患者作为研究对象,随机分为两组各58例。对照组患者采用动力髋螺钉内固定治疗,观察组患者采用股骨近端防旋髓内钉内固定治疗,分析比较两组的治疗效果。结果观察组患者的手术时间及住院时间均显著短于对照组,术中出血量显著少于对照组(均P <0.05)。观察组患者术后3个月、术后6个月的Harris评分均显著优于对照组(均P <0.05)。观察组的术后并发症发生率为3.45%,显著低于对照组的17.24%(P <0.05)。结论股骨近端防旋髓内钉内固定治疗老年骨质疏松性股骨粗隆间骨折的效果显著,可优化手术指标,缩短患者住院时间,且并发症较少。

Objective To analyze the effect of proximal femoral anti-rotation intramedullary nail for internal fixation in the treatment of senile osteoporotic femoral intertrochanteric fracture. Methods 116 cases of elderly patients with osteoporotic femoral intertrochanteric fracture admitted to our hospital from January 2016 to November 2017 were selected as research objects, and randomly divided into two groups equally. The control group was treated with dynamic hip screw for internal fixation, while the observation group was treated with proximal femoral anti-rotation intramedullary nail for internal fixation. The curative effects of the two groups were analyzed and compared. Results The operation time and hospitalization time of the observation group were significantly shorter than those of the control group, and the intraoperative blood loss volume was significantly lower than that of the control group （P〈0.05）. 3 months and 6 months after operation, the Harris scores of the observation group were significantly better than those of the control group （P 〈0.05）. The incidence of postoperative complications of the observation group was 3.45%, significantly lower than 17.24% of the control group （P 〈0.05）. Conclusions Proximal femoral anti-rotation intramedullary nail for internal fixation in the treatment of senile osteoporotic femoral intertrochanteric fracture has significant efficacy and few complications, which can optimize the surgical indicators and shorten the hospitalization time of patients.
