Iliza r ov技术矫治对成人下肢外伤后遗足踝部马蹄畸形预后的影响

Effect of Ilizarov Technique Correction on Prognosis of Horseshoe Malformation of Foot and Ankle After Lower Limb Trauma in Adults

目的探讨实施Ilizarov技术矫治对成人下肢外伤后遗足踝部马蹄畸形预后的影响。方法 80例成人下肢外伤后遗足踝部马蹄畸形患者均采用Ilizarov技术矫治,观察临床治疗效果。结果所有患者均随访0.5～3年,牵伸时间为(12.57±2.47)周,完成牵伸调节后,患者的畸形均得到基本矫正,踝关节活动度对比手术治疗前增加(11.8±1.3)°;按照ICFSG评分,优15足,良55足,可10足,差0足;患者末次随访时的Kofoed踝关节疼痛评分、 AOFAS踝-后足评分均显著优于治疗前(均P <0.05)。结论成人下肢外伤后遗足踝部马蹄畸形实施Ilizarov技术矫治可使畸形得到有效矫正,并促进其功能恢复,缓解疼痛。

Objective To explore the effect ofllizarov technique correction on prognosis of horseshoe malformation of foot and ankle after lower limb trauma in adults. Methods 80 adults with horseshoe malformation of foot and ankle after lower limb trauma were treated with Ilizarov technique correction, to observe the therapeutic effect of the patients. Results All the patients were followed up for 0.5 - 3 years, with the drawing time of （12.57 ± 2.47） weeks; after finishing the draft regulation, the deformity of patients had basic correction, and the ankle joint activity increased （11.8 ± 1.3） ° than that before surgery. According to the ICFSG score, 15 feet were excellent, 55 feet were good, 10 feet were general and 0 foot was poor. The Kofoed ankle pain score and AOFAS ankle-hind foot score of patients at the last follow-up were better than those before treatment （both P〈0.05）. Conclusions Ilizarov technique correction can effectively correct horseshoe malformation of foot and ankle after lower limb trauma in adults, promote the functional recovery, and relieve pain.
