聚乙二醇电解质溶液2L联合蓖麻油法与聚乙二醇电解质溶液4L法肠道准备的效果比较

Comparative analysis of polyethylene glycol electrolyte solution 2L in combination with castor oil and polyethylene glycol electrolyte solution 4L for colonoscopy preparation

目的比较聚乙二醇(PEG)电解质溶液2L联合蓖麻油法与PEG电解质溶液4L法用于结肠镜检查前肠道准备的效果,以及患者的依从性和不良反应情况。方法采用随机、单盲、前瞻性研究方法,选择2016年6—9月在南京医科大学附属常州第二人民医院行结肠镜检查的患者390例,采用随机数字表法分成两组。PEG 2L+蓖麻油组199例,予蓖麻油40mL和复方PEG电解质溶液2L行肠道准备;PEG 4L组191例,予复方PEG电解质溶液4L行肠道准备。记录结肠镜检查结果,并对肠道准备效果进行波士顿(Boston)肠道准备评分。对两组患者进行问卷调查,比较两组患者的依从性和服药后不良反应。结果两组患者肠道准备过程、结肠镜检查结果和Boston肠道准备评分的差异均无统计学意义(P值均>0.05)。PEG 2L+蓖麻油组患者肠道准备的依从性(100.00%)显著高于PEG 4L组(83.25%,P<0.01),不良反应发生率(29.65%)显著低于PEG 4L组(61.26%,P<0.01)。结论复方PEG电解质溶液2L联合蓖麻油法进行肠道准备的清洁效果和安全性较好,费用相对低,适合推广使用。

Objective To compare the efficacy of bowel-cleansing, patients＇ compliance and side effects of polyethylene glycol electrolyte solution 2 L with castor oil and polyethylene glycol electrolyte solution 4 L for colonoscopy preparation. Methods Single-blind prospective randomized trial was performed. A total of 390 patients undergoing colonoscopy from June to September 2016 were randomized into 2 groups to receive polyethylene glycol electrolyte solution 2 L with castor oil （group A, n = 199） and polyethylene glycol electrolyte solution 4 L （group B, n = 191）, respectively. Colonoscopy results and bowel cleansing efficacy （Boston scale） were recorded. All patients filled in a questionnaire and were interviewed about their compliance to the assigned bowel preparation and side effects. Results There were no significant differences in the colonoscopy results or bowel cleansing efficacy between groups （all P〉0.05）. Patients in group A had higher compliance （100.00% vs. 83.25%） and lower rate of side effects （29.65% vs. 61.26%） than group B （both P〈0.01）. Conclusion Polyethylene glycol electrolyte solution 2 L combined with castor oil has good efficacy and safety in bowel cleansing of colonoscopy, and is ecnomic and worth popularizing.
