新诊断2型糖尿病患者血清丝氨酸蛋白酶抑制剂B1水平与胰岛素抵抗的相关性研究

Relationship between serum serpinB1 level and insulin resistance in patients with newly diagnosed type 2 diabetes

目的探讨新诊断T2DM患者血清丝氨酸蛋白酶抑制剂B1(SerpinB1)与IR的关系。方法选取佳木斯大学附属第一医院门诊及住院新诊断T2DM正常体重(18.5≤BMI<24kg/m^2)患者30例(T2DM NW组),新诊断T2DM超重及肥胖(BMI≥24kg/m^2)患者30例(T2DM OW/OB组),同期正常体重健康体检者30名(NC组)。ELISA检测各组血清SerpinB1。计算胰岛素敏感指数(ISI)、胰岛素抵抗指数(HOMA-IR)与胰岛β细胞功能指数(HOMA-β)。结果 (1)与NC组比较,T2DM NW组和T2DM OW/OB组血清SerpinB1水平降低[(65.02±15.12)vs(53.30±12.36)vs(44.58±13.11)ng/ml,P<0.05]。(2)Pearson相关分析显示,血清SerpinB1水平与HDL-C、ISI、HOMA-β呈正相关(r=0.305、0.433、0.371,P<0.05),与体重、BMI、WC、臀围(HC)、WHR、FPG、TG、TC、LDL-C、HbA1c、FIns、HOMA-IR呈负相关(r=-0.271、-0.340、-0.351、-0.260、-0.410、-0.459、-0.340、-0.357、-0.374、-0.474、-0.363、-0.496,P<0.05)。(3)多元线性回归分析结果显示,HOMA-IR、LDL-C是SerpinB1的影响因素。结论新诊断T2DM患者血清SerpinB1水平降低,SerpinB1与IR、LDL-C呈负相关。

Objective To explore the relationship between serum serpinB1 level and insulin resistance（IR）in patients with newly diagnosed type 2 diabetes（T2DM）. Methods A total of 90 subjects were enrolled in this study and divided into three groups：Patients with newly diagnosed T2DM and 18.5≤BMI24 kg/m^2（T2DM NW group,n=30）,patients with newly diagnosed T2DM and BMI≥24 kg/m^2（T2DM OW/OB group,n=30）,and healthy subjects with normal weight and 18.5 kg/m^2≤BMI24 kg/m^2（NC group,n=30）.Serum serpinB1 was measured by ELISA,ISI,HOMA-IR and HOMA-βwere calculated. Results（1）The serum serpinB1 were lower in T2DM NW group and T2DM OW/OB group than in NC group[（65.02±15.12）vs（53.30±12.36）vs（44.58±13.11）ng/ml,P〈0.05].（2）Pearson correlation analysis showed that serpinB1 level was negatively correlated with weights,BMI,WC,HC,WHR,FPG,TG,TC,LDL-C,HbA1 c,FIns and HOMA-IR（r=-0.271,-0.340,-0.351,-0.260,-0.410,-0.459,-0.340,-0.357,-0.374,-0.474,-0.363,-0.496,P〈0.05）,and positively correlated with HDL-C,ISI,and HOMA-β（r=0.305,0.443,0.371,P〈0.05）.（3）Multiple stepwise regression analysis showed that HOMA-IR and LDL-C were the independent influencing factors for serpinB1. Conclusion Serum serpinB1 decreased in patients with newly diagnosed T2DM.Serum serpinB1 is negatively correlated with IR and LDL-C.
