BIRC5、NEIL2表达水平与鼻咽癌放疗敏感性的相关性 被引量:4

Correlation study between the expressions of BIRC5,NEIL2 and the radiotherapy sensitivity of nasopharynx cancer

摘要 目的探讨含重复凋亡结构杆状病毒抑制剂5(BIRC5)、内切核酸酶Ⅷ样蛋白2(NEIL2)与鼻咽癌(NPC)患者放疗敏感性的关系。方法选取2012年4月-2014年4月于本院诊治的NPC患者56例(NPC组),选取同期鼻咽炎患者50例作为对照组。根据随访结果,将NPC患者分为放疗敏感组(n=42)和放疗不敏感组(n=14),采用免疫组化SP法检测病理组织BIRC5、NEIL2水平,比较二者在NPC组和对照组中差异,以及在NPC患者中治疗前后的变化。结果与对照组比较,NPC组患者BIRC5、NEIL2相对表达量均明显升高,差异有统计学意义(P<0.05)。放疗前敏感组患者BIRC5、NEIL2相对表达水平显著低于不敏感组(P<0.05),放疗后敏感组患者NEIL2相对表达水平明显低于不敏感组(P<0.05)。BIRC5、NEIL2预测NPC放疗敏感性ROC曲线下面积分别为0.839、0.817,敏感性分别为79.12%、78.77%,特异性分别为79.53%、79.86%,约登指数分别为0.587、0.586。BIRC5低表达者3年存活率明显高于BIRC5高表达者(94.87%vs. 70.59%,P<0.05);NEIL2低表达者3年存活率明显高于NEIL2高表达者(97.14%vs. 71.43%,P<0.05)。结论 BIRC5、NEIL2在鼻咽癌患者中高表达,且与患者3年生存率相关,对NPC患者放疗敏感性具有一定预测价值。 Objective To detect the expression of baculoviral inhibitor of apoptosis repeat containing 5(BIRC5)and endonuclease Ⅷ-like 2(NEIL2)in nasopharynx cancer(NPC)tissues,and explore their relationship with radiotherapy sensitivity of NPC patients. Methods 56 cases of NPC patients who were treated in our hospital from April 2012 to April2014 were selected as the research subjects. 50 patients with nasopharyngitis were selected as the control group at the same time. According to the follow-up results, NPC patients were divided into radiotherapy sensitive group(n=42) and radiotherapy insensitive group(n=14). Immunohistochemical SP method was used to detect the levels of BIRC5 and NEIL2 in pathological tissues;the differences between them in NPC group and control group were compared,and the changes of two groups in NPC patients before and after treatment were compared. Results Compared with the control group,the relative expression levels of BIRC5 and NEIL2 in nasopharynx cancer tissues were significantly higher than those in the control group(P〈0.05). The relative expression level of BIRC5 and NEIL2 in the sensitive group was significantly lower than that in the non sensitive group before radiotherapy(P〈0.05);the relative expression level of BIRC5 and NEIL2 in the sensitive group after radiotherapy was significantly lower than that in the non sensitive group(P〈0.05). The area under the curve of the BIRC5 and NEIL2 predicted radiotherapy sensitivity of NPC were 0.839 and 0.817,respectively;the corresponding sensitivities were 79.12% and 78.77%,respectively,and the specificities were 79.53% and 79.86% respectively,with the Youden index of 0.587 and 0.586,respectively. The 3 year survival rate of the BIRC5 low expression cases was significantly higher than that of the BIRC5 high expression cases(94.87% vs. 70.59%,P〈0.05). The 3 year survival rate of the NEIL2 low expression cases was significantly higher than that of the NEIL2 high expression cases(97.14% vs. 71.43%,P〈0.05).Conclusion BIRC5 and NEIL2 are highly expressed in nasopharynx cancer patients;the expression levels of BIRC5 and NEIL2 can effectively predict the radiotherapy sensitivity of NPC and the 3 year survival rate of patients with good predictive value for radiotherapy sensitivity of NPC patients.
