影响胃癌腹膜转移患者生存期相关因素分析

Relevant factor analysis on the survival of gastric cancer patients with peritoneal metastasis

目的:回顾性分析胃癌腹膜转移患者临床特征及预后相关性。方法:收集66例胃癌腹膜转移患者的临床资料,分析临床病理特征及治疗对生存期影响。结果:66例胃癌腹膜患者中位生存时间10. 23个月(95%CI:8. 24~12. 22),发生胃癌腹膜转移合并腹腔积液患者中位生存时间9. 4个月,57例患者接受全身化疗,接受全身化疗者中位生存时间12. 13个月,21例患者接受腹腔灌注治疗,其中位生存时间16. 17个月,单因素分析显示胃癌腹膜患者中ECOG评分<2分(P=0. 037),无腹腔积液(P=0. 03),接受全身化疗(P <0. 001)或腹腔灌注治疗(P <0. 001)与预后有关,多因素分析显示是否接受全身化疗(P=0. 01)及腹腔灌注治疗(P=0. 003)是影响胃癌腹膜患者生存期的独立预后因子。结论:胃癌发生腹膜转移时预后较差,生存期短,接受系统化疗联合腹腔灌注治疗,可对胃癌腹膜转移患者带来生存获益。

Objective Rretrospectively analyzed the clinical features and prognosis of gastric cancer patients with peritoneal metastasis. Method The data of 66 gastric cancer patients with operitoneal metastasis were collected to analyze the influence of clinical pathological characteristics and treatment on the survival time. Results The median survival time was 10. 23 months （95% CI：8.24 -12. 22）. The time on the gastric patients with peritoneal metastasis and seroperitoneum was 9.4 months. The median survival time of 57 patients who received chemotherapy was 12. 13 months, while 21 patients received peritoneal perfusion was 16. 17 months. Univariate analysis showed that ECOG score 〈 2 （ P = 0. 037）, without seroperitoneum （ P = 0. 03 ）, chemotherapy （ P 〈 0. 001 ）, intraperitoneal treatment （ P 〈 0. 001 ） were significant factors for prognoise. Multivariate analysis revealed that chemotherapy （ P = 0. 01 ）, intraperitoneal treatment （ P = 0. 003 ） were indenpendt protectibe factors. Conclusion The prog- nosis of gastric cancer is poor in peritoneal metastasis patients. The survival tittle is short. Systematic chemotherapy combined with peritoneal peffusion treatment may bring survival benefits to gastric cancer patients with peritoneal metastasis.
