灯盏花素联合奥美拉唑治疗急性胰腺炎55例临床观察

Clinical Observation on Breviscapine Combined with Omeprazole in the Treatment of 55 Patients with Acute Pancreatitis

目的探讨灯盏花素联合奥美拉唑治疗急性胰腺炎(AP)的临床疗效。方法选择医院2014年5月至2017年5月收治的AP患者109例,按随机数字表法分为对照组(54例)和治疗组(55例)。两组患者均给予奥美拉唑治疗,治疗组患者加用灯盏花素,两组均连续治疗14 d。结果治疗组总有效率为94. 55%,明显高于对照组的79. 63%(χ2=5. 415,P=0. 020);治疗后,两组患者胃肠道功能评分,白细胞介素6(IL-6)、白细胞介素10(IL-10)、肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)、外周血D-乳酸、二胺氧化酶水平及尿乳果糖/甘露醇(L/M)值均明显低于治疗前,且治疗组明显低于对照组(P <0. 05);两组患者治疗过程中均无严重不良反应发生。结论灯盏花素联合奥美拉唑治疗AP临床疗效显著,能明显改善肠黏膜屏障功能,减轻炎性反应,且安全性较好,值得临床推广。

Objective To investigate the clinical effect of breviscapine combined with omeprazole in the treatment of acute pancreatitis（AP）. Methods Totally 109 patients with AP admitted to our hospital from May 2014 to May 2017 were selected and divided into the con- trol group（n =54） and the treatment group（ n =55） according to the random number table method. The two groups were given omepra- zole for treatment,on this basis,the treatment group was treated with breviscapine. The two groups were treated for 14 d. Results The total effective rate of the treatment group was 94.55% ,which was significantly higher than 79.63% of the control group（x2=5.415, P = 0. 020）. After treatment, the scores of gastrointestinal function, the levels of IL - 6, IL - 10, TNF - α, CRP peripheral blood D - lactate,diamine oxidase and tactulose/mannitol（L/M） in the two groups were significantly lower than those before treatment,and those in the treatment group were significantly lower than those in the control group（P 〈 0.05）. There was no serious adverse reactions in the two groups during the treatment. Conclusion Breviscapine combined with omeprazole in the treatment of AP has significant clinical ef- fect,it can significantly improve the intestinal mucosal barrier function, reduce inflammatory reaction,it is safe and worthy of clinical promotion.
