牙周基础治疗对2型糖尿病伴慢性牙周炎患者血清生化指标及血糖代谢水平的影响

The effects of periodontal basic therapy on serum biochemical indexes and blood glucose metabolism of the patients with type 2 diabetes and periodontitis

目的:探讨牙周基础治疗对2型糖尿病伴慢性牙周炎患者血糖代谢水平及血清生化指标的影响。方法:纳入2型糖尿病伴慢性牙周炎患者52例,对所有患者进行牙周基础治疗,比较治疗前及治疗后4周菌斑指数(PLI)、探诊深度(PD)、探诊出血指数(BOP)、出血指数(BI)、附着丧失(CAL)的变化,并对比血糖代谢及血清生化指标的变化情况。结果:与治疗前相比,牙周基础治疗后4周,患者各项牙周临床指标均显著降低(P <0. 05);糖代谢及血清生化指标中,空腹血糖水平未见明显改善(P> 0. 05);血清钙磷比、甘油三酯、糖化血红蛋白明显降低(P <0. 05);总胆红素、载脂蛋白A1明显升高(P <0. 05)。结论:牙周基础治疗能改善2型糖尿病伴慢性牙周炎患者血清生化指标及血糖代谢水平,并可有效减轻牙周炎症。

AIM： To investigate the effects of periodontal basic therapy on blood glucose metabolism and serum biochemical indexes of the patients with type 2 diabetes and periodontitis. METHODS： 52 cases of type 2 diabetes mellitus patients with chronic periodontitis were given periodontal basic therapy, before and after treatment the periodontal clinical parameters of plaque index（PLI）, periodontal pocket depth（PD）, bleeding on probing （BOP）,bleeding index（BI）and clinical attachment loss（CAL） were measured. The blood glucose metabolism and serum biochemical indexes were measured. RESULTS： 4 weeks after initial periodontal treatment, the periodontal indexes were decreased（ P 〈0.05）;the level of HbAlc decreased （ P 〈0.05）; the level of FBG was not obviously changed（ P 〉0.05 ）;serum phosphorus, calcium and phosphorus ratio, triglyceride decreased （ P 〈0.05）; T-Bil and ApoA1 increased （ P 〈0.05）. CONCLUSION： Periodontal basic therapycan improve serum calcium and phosphorus ratio, blood lipid metabolism, glycosylated hemoglobin and periodontal parameters in patients with type 2 diabetes mellitus and periodontitis.
