膀胱外翻患儿的围手术期护理体会

Perioperative nursing management of one-stage reconstruction for bladder exstrophy in children

目的总结膀胱外翻患儿I期修复手术的护理经验。方法回顾性分析2014年11月—2017年12月期间收治的5例膀胱外翻患儿的病例资料,总结临床护理经验。结果 5例患儿均予以I期膀胱外翻手术治疗,并在围手术期予以针对性护理,术后有1例患儿在出院后出现骶尾部压疮,经过清创处理恢复良好,余4例患儿均未出现导管感染、压疮、伤口开裂、切口感染等严重并发症。随访2~12个月后,其中4例患儿获得功能性控尿,1例患儿出现尿失禁。结论围手术期针对性护理对膀胱外翻患儿的病情恢复有积极意义,同时应加强护理宣教,尽早发现和处理相关并发症,改善患儿生活质量,提高手术成功率。

Objective To summarize the perioperative nursing management of one-stage recon- struction for bladder exstrophy in children. Methods This study included 5 pediatric patients who received one-stage reconstruction for bladder exstrophy between November 2014 and December 2017. Their clinical data and nursing management were retrospectively analyzed. Results All pa- tients received one-stage reconstruction and targeted nursing interventions during the operation peri- od. There were no severe complications in 4 cases except one patient who had sacrococcygeal pres- sure ulcers. After 2 to 12 months of follow-up, 4 patients had functional control of urine and 1 pa- tient had urinary incontinence. Conclusion Perioperative nursing care has an important role in the recovery of children one-stage reconstruction for bladder exstrophy. It can effectively prevent the occurrence of postoperative complications and significantly improve postoperative life quality.
