2014—2016年上海市公立医院经济运行状况分析

Analysis on operating economical status of public hospitals in Shanghai from 2014 to 2016

目的分析2014—2016年上海市不同类型公立医疗机构的经济运行及财政补偿情况,为上海市公立医院的改革提供决策参考和依据。方法采用描述性分析,对2014—2016年上海市卫生财务年报数据进行分析。结果2014—2016年上海市公立医院总体上收支平衡,药品在医院收入中占比逐年下降,取消药品加成政策在上海并未取得理想的效果。医疗服务价格调整对公立医院收支结构产生了良性影响,但不同类型、不同级别的医院收支变化差异大。结论公立医院需进一步贯彻药品零差率政策,财政应进一步加大对公立医院政策性亏损的补偿力度,并建立完善不同级别和类型的公立医院的动态补偿机制。

Objective To analyze operating economical status and financial compensation of different types of publicmedical institutions in Shanghai from 2014 to 2016,and to provide references for policy making on public hospitals reformin Shanghai. Methods A descriptive analysis was conducted for data mining between 2014 and 2016 from Shanghai HealthFinancing Annual Reports. Results From 2014 to 2016,financial budgets of public hospitals in Shanghai were generallybalanced. The share of pharmaceutical income in total income of hospitals was decreasing year by year,and the effect ofelimination drug markup wasn’t ideal. The price adjustment of medical services had a positive impact on financial structureof public hospitals,while there were significant differences among different types and levels of hospitals. Conclusion It issuggested that public hospitals further implement policy on the drug markup elimination. Meanwhile,the government isproposed to further increase the financial compensation for policy-related losses of public hospitals and improve the dynamiccompensation mechanism for different levels and types of public hospitals.
