Comprehensive evaluation on the computed tomography utilization in Sichuan Province

摘要 目的了解和评价2015年四川省计算机断层扫描(computed tomography,CT)设备利用情况,为大型医用设备的规划配置和管理提供参考。方法对四川省所有拥有大型医用设备的医疗机构进行问卷调查,采用开机利用率、能力利用率、阳性率、诊断一致率4个指标对CT设备的利用效率进行描述性分析,并引入熵权与理想方案相似性的顺序选优技术(technique for order preference by similarity to ideal solution,TOPSIS)法对各市(州)设备利用情况进行综合评价。结果 CT设备的开机利用率、能力利用率与医疗机构等级和地区经济水平呈正相关,CT设备的阳性率、诊断一致率与自评调查的开展情况呈正相关,综合4个指标进行熵权TOPSIS法排名发现,经济水平高的市(州)CT设备的综合利用情况不一定好,不同市(州)间的设备利用情况差异较大。结论存在CT设备的利用情况参差不齐,区域规划不合理,部分医院CT设备使用必要性不足的问题,引入熵权TOPSIS法可以更好地综合各利用指标,得到更科学全面的结果。迫切需要建立健全大型医用设备配备的相关机制,加强CT设备的使用和管理,提高其经济和社会效益。 Objective To understand and evaluate the utilization of computed tomography(CT) in Sichuan Provincein 2015 and provide references for planning,configuration and management of large-scale medical equipment. MethodsQuestionnaire surveys were conducted on all medical institutions with large-scale medical equipment in Sichuan Province.The efficiency of CT utilization was described with four indexes the power utilization rate,the capacity utilization rate,the positive rate and the diagnostic consistency rate. The method of entropy technique for order preference by similarity toideal solution(TOPSIS) was introduced to cities or states for a comprehensive evaluation on equipment utilization. ResultsCT power utilization and capacity utilization rate were found to be positively correlated to medical institution level andregional economic level. CT positive rate and diagnostic consistency rate were positively correlated with the development ofself-evaluation surveys. Four indexes were used in ranking with the application of the entropy TOPSIS method. Cities andstates with high economic levels did not necessarily have good comprehensive utilization of CT,and there were significantdifferences in the use of equipment among different cities and states. Conclusion There are some problems such as unevenutilization of CT,unreasonable regional planning and insufficient necessity of CT use in some hospitals. The introduction ofentropy TOPSIS method can better synthesize various utilization indexes to obtain more scientific and comprehensive results.There is an urgent need to establish and improve relevant mechanisms for the deployment of large-scale medical equipment,strengthen the use and management of CT,and increase economic and social benefits.
