当前全世界各个国家卫生管理组织非常关注的热点问题之一就是非惩罚性医疗安全(不良)事件报告系统的构建和应用。研究调查显示,医疗不良事件的报告是促进患者安全和保证质量改进的重要举措和方法,对医疗质量的提升具有其重大意义。首先对国外包括美国、英国、加拿大、日本等典型国家成熟的医疗不良事件报告系统构建状况进行综述;也对国内的不良事件上报管理机制进行具体阐述,目前各级医疗机构重视该项工作,有国家卫计委的报告系统、各级医疗机构内部的报告系统、医疗器械不良事件的报告系统、药品不良反应事件的报告系统、香港地区的医管局医事汇报系统五个上报系统在成熟运行。同时提出强化国家卫生行政部门机制建设和督导落实的结合、强化国家与各级医疗机构的内部系统的结合、强化不良事件报告人为医务人员和患者及家属的结合、强化不良事件报告独立性和保密性非惩罚性的结合、强化不良事件报告系统的自愿性与强制性的结合,以保证医疗安全(不良)事件报告管理体制的顺利运行。
