江西≥50岁男性HIV/AIDS病人确诊前后高危行为研究

High risk behaviors of the elderly male HIV/AIDS before and after diagnosis in Jiangxi province

目的调查江西省老年男性艾滋病病毒（HIV）感染者/艾滋病（AIDS）病人（简称HIV/AIDS病人）确诊前后的高危行为,为制定有针对性的防治策略和干预措施提供参考依据。方法采用横断面调查,设计调查问卷,对江西省≥50岁男性HIV/AIDS病例进行确诊前后的高危行为调查。结果共调查了711例≥50男性HIV/AIDS病例,其中确诊前有异性商业性行为521例,消费场所为路边店、出租屋的413例,占79.27%,付费价格20-49元的246例,占47.22%。艾滋病知识知晓率为72.71%（517例）,壮阳药使用的比例为42.48%（302例）。发生商业异性性行为比例分别由确诊前的73.28%降至确诊后的7.59%,与配偶或固定性伴发生性行为坚持使用安全套比例由23.98%提高至63.08%,商业性行为坚持使用安全套比例由1.54%提高至62.96%。结论老年HIV/AIDS病人确诊后商业性行为发生率有大幅度下降,坚持使用安全套比例有所升高。应继续加强对老年HIV/AIDS病例的宣传教育和干预,提高安全套使用率,减少艾滋病二代传播。

Objective To understand the high risk behaviors before and after diagnosis of elderly HIV/AIDS patients,and provide scientific evidence for targeted prevention and intervention.Methods A cross-sectional survey was conducted on the high-risk behavior of HIV/AIDS male patients over 50 years old before and after HIV diagnosis in Jiangxi province.Results A total of 711 elderly male HIV/AIDS cases participated in this study.Among them,521 cases had heterosexual commercial sex behavior before diagnosis.79.27% had commercial sex with street girls or the FSW in rental houses,with the expenses of RMB 20-49 yuan.Results showed that their awareness rate of AIDS knowledge was 72.71%,and 42.48% used aphrodisiac.The proportions of those with commercial heterosexual behaviors decreased from 73.28% before diagnosis to 7.59% after diagnosis.The rate of condom use with spouses or fixed partners increased from 23.98% to 63.08%,and the rate of condom use in their commercial sexual behaviors increased from 1.54% to 62.96%. Conclusion There is a significant decline in the incidence of commercial sex after HIV diagnosis among the elderly.The rate of condom use is also increasing.Publicity and education about AIDS knowledge should be enhanced among the elderly,to promotion condom use and reduce the secondary AIDS transmission.
