摘要
目的观察艾滋病病毒1型(HIV-1)合并带状疱疹病毒(VZV)感染者,在高效抗反转录病毒治疗(HAART)前后外周血中HIV-1脱氧核糖核酸(DNA)水平的变化。方法选取慢性期HIV-1合并VZV感染者11例作为试验组(VZV+),46例未合并VZV感染者作为对照组(VZV-)。对0周和抗病毒治疗48周时感染者外周血进行HIV-1DNA的定量检测。结果基线时,VZV+组HIV-1DNA水平为(3.40±0.36)Log10拷贝/10^6外周血单个核细胞(PBMCs),明显高于VZV-组的(3.04±0.44)Log10拷贝/10^6 PBMCs(P=0.014)。抗病毒治疗48周后,VZV+组HIV-1DNA水平为(3.19±0.33)Log10拷贝/10^6 PBMCs,较基线时未出现明显的下降(P=0.182);VZV-组HIV-1DNA水平为(2.56±0.46)Log10拷贝/10^6 PBMCs,较基线时明显下降(P〈0.001),并且显著低于VZV+组(P〈0.001)。治疗前和治疗后的HIV-1DNA水平均与基线病毒载量呈正相关,与基线CD4细胞计数、辅助性T细胞/抑制性T细胞比例呈负相关。结论 HIV-1合并VZV感染者具有较高的基线HIV-1DNA,并且在有效地抗病毒治疗之后仍然高于未合并VZV感染者。
Objective To observe the impact of varicalle-zoster virus(VZV)coinfection on the decay of HIV-1 DNA in HIV-1 infected patients before and after highly active antiretroviral therapy(HAART).Methods 11 HIV-1 chronically infected patients co-infected with VZV were recruited as the study group and 46 HIV-1 infected patients without co-infection as controls.HIV-1 DNA was quantified with peripheral blood at week 0 and 48.Results Compared with the VZV-group,the baseline HIV-1 DNA of the VZV+ group was much higher(3.40±0.36 vs3.04±0.44 Log10 copies/10^6 PBMCs,P=0.014).At week 48 on HAART,the level of HIV-1 DNA of the VZV+group was 3.19±0.33 Log10 copies/10^6 PBMCs,without significant decrease(P=0.182).As for the VZV-group,HIV-1 DNA dramatically decayed to 2.56±0.46 Log10 copies/10^6 PBMCs,much lower than baseline level(P〈0.001)and the VZV+ group(P〈0.001).Both the levels of HIV-1 DNA at baseline and after treatment were positively correlated to baseline viral load,but negatively correlated to CD4 cell count and Th/Ts ratio.ConclusionHIV-1 patients co-infected with VZV present higher HIV-1 DNA level no matter before or after effective HAART.
