西酞普兰联合米氮平对重度抑郁症伴睡眠障碍患者的睡眠质量、生活质量及氧化应激水平的影响

Effect of mirtazapine combined with escitalopram oxalate on sleep quality,quality of life and oxidative stress levels of patients with major depressive disorder complicated with sleep disorder

目的探讨西酞普兰联合米氮平对重度抑郁症伴睡眠障碍症患者睡眠质量、生活质量及氧化应激水平的影响。方法选取2015年1月~2017年1月于本院诊治重度抑郁症伴睡眠障碍症患者60例,根据随机数字法将患者分成联合组和单药组,各30例,单药组给予西酞普兰治疗,联合组在此基础上给予米氮平治疗,均连续治疗6周,对比分析两组患者治疗前后睡眠质量、生活质量及氧化应激平变化情况。结果治疗前两组患者睡眠中质量、生活质量、氧化应激水平均无明显差异(P>0.05);治疗后,两组患者PSQI 7个因子(评分及总分较治疗前均降低,联合组患者评分均低于单药组,生活质量量表中的总症状评分及日常生活评分均高于单药组,超氧化物歧化酶及一氧化氮水平均高于单药组,丙二醛及血浆内皮素-1低于单药组,上述差异均有统计学意义(P<0.05)。结论西酞普兰联合米氮平能有效提高重度抑郁症伴睡眠障碍症患者睡眠质量、生活质量,并降低体内的氧化应激水平,值得临床作进一步推广。

Objective To discuss the effect of mirtazapine combined with escitalopram oxalate on sleep quality,quality of life and oxidative stress levels of patients with major depressive disorder and sleep disorder s. Methods 60 patients with major depressive disorder complicated with sleep disorder were selected from January 2015 to January 2017 in our Hospitals. According to the random number method,all the patients were divided into observation group（30 cases）and control group（30 cases）,the observation group were given mirtazapine combined with escitalopram oxalate,while the control group were given mirtazapine only. All patients were treated for 6 weeks. The quality of sleep,quality of life and the change of oxidative stress in two groups were compared and analyzed before and after treatment. Results There was no significant difference in the quality,quality of life,and oxidative stress levels in the two groups before therapy（P〈0.05）. After treatment,PSQI seven factors scores and total scores of two groups of patients were all decreased,and the parameters of the observation group were lower than those of the control group,the total symptom scores and daily life scores in QOL of the observation group were higher than those of the control group,The levels of superoxide dismutase and nitric oxide of the observation group were higher than those of the control group,the hyperpropylene dialdehyde and plasma endothelial–1 of the observation group were lower than those of the control group,the difference mentioned above were all statistically significant（P〈0.05）. Conclusion Mirtazapine combined with escitalopram oxalate can effectively improve the sleep quality,quality of life of patients with major depressive disorder complicated with sleep disorder and reduce the oxidative stress levels as well,thus,it＇s worth for further clinical promotion.
