MRI在早产儿缺氧缺血性脑病中的诊断价值

The Diagnostic Value of MRI in Premature Infant with Hypoxic Ischemic Encephalopathy

目的:探讨磁共振成像(MRI)在早产儿缺氧缺血性脑病(HIE)中的诊断价值。方法:选取2015年1月—2018年1月HIE早产儿100例,结合院外及院内CT、MRI检查,比较早产儿CT、MRI检查的诊断结果。结果:MRI脑水肿、脑室出血检出率明显高于CT,MRI蛛网膜下腔出血检出率明显低于CT,差异有统计学意义(P<0.05),CT、MRI脑动脉梗死、基底节出血、矢状旁区损伤检出率基本相同,差异无统计学意义(P>0.05);MRI病变检出率、诊断符合率、病情分度评估准确率明显高于CT,差异有统计学意义(P<0.05)。结论:与CT比较,MRI在早产儿HIE中具有更高的诊断和病情分度评估价值,但CT对蛛网膜下腔出血检出率更高,可作为辅助诊断。

Objective： To discuss the diagnostic value of magnetic resonance imaging （MRI） in premature infact with hypoxic ischemic encephalopathy （HIE）. Methods： 100 premature infact with HIE were selected from January 2015 to January 2018, the diagnosis results of CT and MP.I of premature infants were compared in combination with the CT and MRI examinations outside and inside the hospital, Results： The cerebral edema, cerebral ventricular hemorrhage detection rate of MRI examination were significantly higher than those of the CT examination, the subarachnoid hemorrhage detection rate of MRI examination were significantly lower than those of the CT examination, the difference was statistically significant （P〈0.05）, the cerebral arterial infrtion, basal ganglia hemorrhage, sagittal collateral damage detection rate of CT and MRI examinatiou were basically the same, the difference was not statistically significant （P〉0.05）. The disease detection rate, diagnostic compliance rate, disease degree cwduation accuracy rate of MRI examination were significantly higher than those of the CT examination, the difference was statistically significant （P〈0.05）. Conclusion： Compare with CT, MRI has higher diagnostic and illness deg-ree evaluation value in premature in^mt with HIE. but CT examination ofsubarachnoid hemorrhage detection rate is higher, it can be used as auxiliary, diagnosis.
